Acute myocardial infarction (ami) is one of the most prevalent and critical clinical syndromes requiring appropriate risk characterization and biomarker analysis for adequate management. cardiac biomarkers, such as troponin i, creatine kinase-mb (ck-mb), and n-terminal pro-b-type natriuretic peptide (nt-probnp), are important in diagnosing and risk-stratifying ami. however, the effect of body mass index (bmi) on these biomarkers has not been evidenced, especially among critically patients. the aim of this study was to examine the relationship between bmi and cardiac biomarkers (troponin i, ck-mb, and nt-probnp) among type ii non-st-elevation myocardial infarction (nstemi) patients admitted to the intensive care unit (icu) to improve diagnosis and risk stratification. a case-control study was conducted using 39 type ii nstemi patients and 19 controls. bmi and the levels of cardiac biomarkers were collected, and the pearson correlation coefficient was conducted to analyze their relationship. a significant positive correlation was observed between bmi and nt-probnp (r = 0.654, p < 0.001), while no significant correlation was found between bmi and troponin i (r = 0.1, p = 0.546) or ck-mb (r = 0.051, p = 0.756). in conclusion, bmi affects nt-probnp levels; therefore, the thresholds should be adjusted based on the bmi. however, troponin i and ck-mb retained their prognostic values in cases of myocardial involvement regardless of bmi values. these results highlighted the neccesity of more accurate individual biomarker profiling of ami patients.
