چکیده    Superior mesenteric artery (sma) syndrome refers to duodenal obstruction caused by compression between the aorta and sma. surgical intervention becomes necessary if conservative measures are ineffective. the aim of this study was to assess the outcome of duodenojejunostomy as a management modality of smas.this is prospective study included 10 patients who underwent open duodenojejunostomy at al-zahraa university hospital between january 2022 and december 2024. all patients met predefined inclusion criteria, including radiological confirmed smas, failure of conservative management, and surgical eligibility. data were collected prospectively, including demographic details, preoperative symptoms, operative findings, and postoperative outcomes. follow-up was conducted at 1, 3, 6, and 12 months postoperatively.ten patients (2 males and 8 females) were included. the patients were in the age-range of 16 to 58 years with mean age 24.2±10.8 years, the mean preoperative bmi was 17.11±2.9. the most frequent symptoms at time of presentation were abdominal pain (80%), nausea, and vomiting (70%), and weight loss (60%). a successful outcome was defined as either resumption of solid food without postprandial discomfort, discontinuation of parenteral nutrition, or sustained weight gain of at least 5% from baseline over the 12-month follow-up period. based on these criteria, 8 out of 10 patients (80%) achieved clinical success following open duodenojejunostomy surgery is the recommended treatment for superior mesenteric artery syndrome (smas), a rare disorder that necessiates a high degree of suspicion for diagnose, particularly in females with low body mass index and upper gastrointestinal symptoms, to enhance symptoms and quality of life.
