pregnancy complicated by uterine myoma: a two-year case series from a tertiary referral hospital in indonesia

Background and objective: uterine myomas are prevalent benign neoplasms in women of reproductive age. complicated pregnancies necessitate meticulous management due to potential dangers to both the mother and fetus. this study aimed to evaluate pregnancy outcomes associated with uterine myomas in a tertiary hospital in indonesia. materials and methods: this retrospective study examined the medical records of pregnant women with uterine myomas who gave birth at dr. soetomo hospital from january 2023 to december 2024. demographic, obstetric, and perinatal outcome data were evaluated. myoma types were analyzed using chi-squared or fisher’s exact tests, with p < 0.05 considered significant. results: of 2,067 deliveries, 24 pregnancies (prevalence 1.16%) involved uterine myomas. intramural myomas (figo type 4) predominated, mostly located in the uterine corpus, ranging 2 to >15 cm in size. preterm labor was the most frequent antepartum complication (58.3%), followed by fetal malpresentation (33.3%). postpartum hemorrhage (pph) occurred in 12.5%, and blood transfusion was similarly frequent postpartum. neonatal complications included mild birth asphyxia (62.5%) and nicu admission (16.7%). cesarean section was the primary delivery method (83.3%), and myomectomy was concurrently performed in 41.7% of cases. statistically significant complications included abortion, postpartum sepsis, and severe birth asphyxia, particularly associated with uterine fibroids. conclusion: uterine myomas during pregnancy elevate the risk of premate labor, malpresentation, newborn hypoxia, and postpartum infections. efficient management necessitates prompt identification, vigilant oversight, and tailored multidisciplinary intervention to enhance maternal and newborn outcomes.
