   a comparative study of laparoscopic cholecystectomy after ercp with or without biliary stenting in patients with calculous obstructive jaundice

چکیده    Calculous obstructive jaundice is most commonly caused by stones in the cbd. standard treatment involves ercp for stone removal, followed by (lc). this work aims to assess the outcomes of lc after ercp with and without cbd stenting regarding operative difficulties and times, intraoperative and early postoperative complications. methods: a prospective study was conducted at alzahraa university hospital between june 2022 and june 2024, involving 60 patients diagnosed with calculous cholecystitis and cbd stones. all patients underwent successful ercp and sphincterotomy before laparoscopic cholecystectomy. the patients were randomly divided into two equal groups: group a (n = 30) underwent lc after ercp with biliary stenting, and group b (n = 30) underwent lc after ercp without biliary stenting results: the mean age was 48.6 ± 9.2 years in group a and 49.3 ± 10.1 years in group b. the male-to-female ratio was 14:16 in group a and 13:17 in group b (33.3%). in group a, there were diabetics, compared to (30%) in group b. the mean operative time in group a was 71.8 ± 14.6 minutes. 23.3% experienced difficulties during dissection at calot’s triangle. 0.13% were converted to open cholecystectomy (oc), and(3.3%) had a bile duct injury. (6.7%) were converted to oc, and no cases of bile duct injury were reported. regarding early postoperative complications, no cases of wound infection or biliary leak were reported in group a. in contrast, in group b, (3.3%) developed a wound infection, and (6.7%) experienced biliary leaks. hospital stays were shorter in group a (2.3 ± 0.6 days) compared to group b (3.9 ± 1.2 days), as patients in group b who developed biliary leaks required ercp and stent placement.
