بررسی تاثیر دکسمدتومدین وریدی بر میزان خونریزی، رضایت جراح و ثبات همودینامیک در بیماران تحت عمل جراحی سپتورینوپلاستی: راهنمای کارآزمایی بالینی

مقدمه: در جراحی رینوپلاستی، عوامل زیادی در به دست آمدن نتیجۀ مطلوب موثر هستند که از جملۀ این عوامل می توان به دقت و مشاهدۀ جراح، آنالیز آناتومی موجود، انتخاب تکنیک جراحی، درجۀ ترومای نسج نرم و غضروف، میزان خونریزی، کیفیت ابزارهای جراحی و بیهوشی و هرچیزی که درکاهش خونریزی موثر بوده، اشاره کرد؛ بنابراین پژوهش حاضر، با هدف بررسی تاثیر دکسمدتومدین وریدی بر میزان خونریزی، رضایت جراح و ثبات همودینامیک در بیماران تحت عمل جراحی سپتورینوپلاستی انجام شد.روش کار: پژوهش حاضر، به روش کارآزمایی بالینی تصادفی شدۀ دو سویۀ کور بود که بر روی 50 بیمار 15 تا  65 سال (در دو گروه 25 نفره) در کلاس بیهوشی(asa) 1 و 2 بیماران تحت عمل جراحی سپتورینوپلاستی که تحت بیهوشی عمومی در بیمارستان مطهری جهرم بودند، انجام گرفت. بیماران به طور تصادفی به دو گروه کنترل(شاهد= پلاسبو) و  مداخله(دکسمدتومیدین)  تقسیم شدند.  علایم حیاتی، آرام بخشی، میزان درد، خونریزی و رضایت جراح، مورد ارزیابی قرار گرفت. تجزیه وتحلیل اطلاعات، به وسیلۀ شاخص های آمار توصیفی(میانگین، درصد، تعداد و انحراف معیار) و استنباطی با استفاده از نرم افزار spss ورژن 21 انجام شد. سطح معناداری p<0.05 درصد، در نظر گرفته شد.بحث: پژوهش حاضر، به بررسی تاثیر دکسمدتومدین وریدی بر میزان خونریزی، رضایت جراح و ثبات همودینامیک در بیماران تحت عمل جراحی سپتورینوپلاستی پرداخت تا با بررسی نتایج این مطالعه و مشخص شدن اثر داروی دکسمدتومدین بتواند، از این دارو در جهت پیشگیری ازتغییرات همودینامیک و کاهش بروز عوارض محتمل در هنگام عمل جراحی سپتوراینوپلاستی استفاه کند.

investigating the effect of intravenous dexmedetomidine on the amount of bleeding, surgeon satisfaction and hemodynamic stability in patients undergoing septorhinoplasty surgery: a study protocol for a randomized controlled trial (rct)

introduction: in rhinoplasty surgery, many factors are effective in obtaining the desired result. among these factors, we can mention the accuracy and observation of the surgeon, the analysis of the existing anatomy, the choice of surgical technique, the degree of soft tissue and cartilage trauma, the amount of bleeding, the quality of surgical instruments and anesthesia, and everything that is effective in reducing bleeding. therefore, this study conducted with the aim of investigating the effect of intravenous dexmedetomidine on the amount of bleeding, surgeon satisfaction and hemodynamic stability in patients undergoing septorhinoplasty surgery.methods: this double-blind randomized clinical trial study conducted on 50 patients aged 15 to 65 (in two groups of 25) with anesthesia class 1 and 2, patients undergoing septorhinoplasty surgery under general anesthesia at motahari hospital in jahrom. patients were randomly divided into two groups (control = placebo) and intervention (dexmedetomidine). vital signs, sedation, level of pain, bleeding and satisfaction of the surgeon were evaluated. data analysis was done by descriptive statistics and inferential statistical tests using spss version 20 software. a significance level of p<0.05% was considered.discussion: this study investigated the effect of intravenous dexmedetomidine on the amount of bleeding, surgeon satisfaction and hemodynamic stability in patients undergoing septorhinoplasty surgery, so that by examining the results of this study and determining the effect of dexmedetomidine, this drug can be used to prevent hemodynamic changes and reduce the incidence of possible side effects during operation in septorhinoplasty surgery.i
