تاثیر اجرای مراقبت پرستاری مبتنی بر نظریه اورلاندو بر اضطراب و تبعیت از درمان بیماران کاندید جراحی برداشتن سینه: پروتکل مطالعه کارازمایی بالینی

مقدمه: به منظور درمان سرطان سینه روش‌های مختلفی از جمله جراحی به کار برده می‌شود. جراحی برداشتن سینه به عنوان بخشی از بدن زن که نماد جنسیت است، منجر به بروز اضطراب در زن‌ها می‌گردد. از آنجایی که پرستار نقش کلیدی در حمایت روانی قبل از عمل بیمار دارد، اقدامات او می‌تواند در کاهش اضطراب و بالا بردن میزان تبعیت از درمان بیماران تاثیر گذار باشد. هدف این مطالعه بررسی تاثیر اجرای مراقبت پرستاری مبتنی بر نظریه اورلاندو بر اضطراب و تبعیت از درمان بیماران کاندید عمل جراحی برداشتن سینه می‌باشد.روش کار: این مطالعه یک کارازمایی بالینی تصادفی شده شاهددار می‌باشد.  نمونه‌ها شامل بیماران مبتلا به سرطان سینه خواهند بود که کاندید جراحی  برداشتن سینه در بخش جراحی بیمارستان فقیهی شیراز هستند. بیماران به صورت تصادفی ساده به قید قرعه در دو گروه مداخله (30 نفر) و کنترل (30 نفر) تخصیص خواهند یافت. شرکت کنندگان در گروه مداخله مراقبت پرستاری مبتنی بر نظریه اورلاندو را دریافت خواهند کرد اما گروه کنترل مراقبت‌های معمول بخش را دریافت خواهند کرد. اضطراب بیماران قبل و بلافاصله بعد از مداخله به وسیله پرسشنامه اضطراب بک و تبعیت از درمان آنها قبل و یک ماه بعد از مداخله به وسیله پرسشنامه مدانلو سنجیده خواهد شد. تجزیه و تحلیل داده‌ها با استفاده از نرم‌افزار اس پی اس اس نسخه 21 در سطح معناداری 0/05 انجام خواهد شد.بحث: این مطالعه تاثیر مراقبت پرستاری مبتنی بر نظریه اورلاندو را بر اضطراب و تبعیت از درمان بیماران کاندید ماستکتومی ارزیابی خواهد کرد.  این مطالعه شواهدی از یک استراتژی برای اجرای مراقبت پرستاری کارآمد و موثر توسط پرستاران را ارائه خواهد داد که در نتیجه آن ممکن است سطوح اضطراب بیماران قبل از جراحی را کاهش داده و میزان تبعیت از درمان آنها بعد از جراحی را نیز افزایش دهد.

the effect of implementing nursing care based on orlando’s theory on anxiety and treatment compliance of patients who are candidates for breast removal surgery (mastectomy): a clinical trial study protocol

introduction: in order to treat breast cancer, various methods including surgery are used.  mastectomy, as a part of the womans body that symbolizes gender, leads to anxiety in women. since the nurses plays a key role in patients psychological support before the operation, their actions can help reduce anxiety and increase the level of adherance to treatment of patients. the purpose of this study is to examin the effect of nursing care based on orlando’s theory on anxiety and treatment adherence of patients who are candidates for mastectomy.methods materials: this study is a randomized controlled clinical trial.  the samples will include patients with breast cancer who are candidates for mastectomy in the surgery department of faghihi hospital in shiraz. the patients will be randomly allocated simply by lottery in two intervention (30 people) and control (30 people) groups.  participants in the intervention group will receive nursing care based on orlando’s theory, but the control group will receive the departments routine care.  the patients’ anxiety before and immediately after the intervention will be measured by the beck anxiety questionnaire and their adherence to treatment before and one month after the intervention will be measured by the madanlo questionnaire.  data analysis will be carried out using the spss software version 21 at a significance level of 0.05.discussion: this study will evaluate the effect of nursing care based on orlando’s theory on the anxiety and adherance to treatment of mastectomy candidate patients. this study will provide evidence of a strategy for the implementation of efficient and effective nursing care by nurses, which may reduce the anxiety levels of patients before surgery and increase their adherance to treatment after surgery.
