مروری بر چالش‌های اخلاقی تجربه‌شده توسط پرستاران ارائه دهنده مراقبت به بیماران مبتلا به کووید 19

مقدمه: با توجه به معضلات اخلاقی شدید ناشی از شیوع کووید 19، و مراقبت‌های مربوط به آن، بررسی، تجزیه و تحلیل این تنش‌ها و معضلات اخلاقی آن بسیار مهم می‌باشد؛ در حالی که یک رویکرد مروری می تواند خود، بینش مهمی در مورد این چالش‌ها ارائه دهد. این تحقیق با هدف جمع‌بندی معضلات و چالش‌های اخلاقی رایج و ظهوریافته در بحران پاندمی کووید 19، صورت گرفته است.روش: پژوهش مروری یکپارچه در پایگاه اطلاعاتیpubmed, google scholar ، web of science وscopus ، براساس mesh و کلیدواژه‌های ذکرشده در مقالات مرتبط و بکارگیری عملگرهای منطقی(or, not, and)، در میان تحقیقات منتشرشده از تاریخ30 زانویه2020 انجام و سپس نتایج از طریق مرور متون واجد معیار ورود، استخراج گردید.یافته‌ها: فقدان مراقبت بیمارمحور و همدلانه، تبعیض، توجه نکردن به استقلال بیماران و مراقبت خانواده‌محور، احساس فقدان مسئولیت‌پذیری، تعارض بین تعهد حرفه‌ای و مسئولیت‌های شخصی خود می تواند از رایج‌ترین معضلات اخلاقی باشد که منجر به آسیب اخلاقی، تبعات روانی، عاطفی، فشار اخلاقی، ناتوانی در سازگاری روانشناختی و ازدست‌دادن احترام بین افراد گردد. بدینگونه مواجهه با این چالش‌ها می‌تواند مهارت‌ها، نوآوری و خلاقیت پرستاران را ارتقاء بخشد. راهبردهای خودمراقبتی، حمایت‌های ساختاری و مداخلات تخصصی باعث می شود که پیامدهای منفی کاسته و پیامدهای مثبت افزایش یابد.نتیجه‌گیری: پرستاران طی مراقبت از بیماران کووید 19، ممکن است در تبدیل تصمیمات اخلاقی نسبت به اجرا و کار اخلاقی دچار مشکل گردند. بنابراین، وقت آن رسیده که در این زمان دشوار، برای مقابله با این چالش ها و حمایت از پرستاران، راهکارهای مقابله‌ای موثر در سطوح مختلف درنظر گرفته شود.

Ethical challenges experienced by nurses providing care to patients with Covid -19

Introduction:It is essential to analyze stresses and ethical problems regarding the severe ethical dilemmas caused by COVID19 and relevant care. In this case, a review approach provides an outstanding insight into this topic. The extant research aimed at reviewing the common and emerged ethical challenges and dilemmas during the COVID19 pandemic. Methods and Materials:An integrative review was done through the database (e.g., Google Scholar, PubMed, Web of Science, and Scopus) based on the Mesh and keywords mentioned in associated articles. This study was conducted by using logical operators (AND, NOT, OR) among the researches published since 30 January 2020. In the next step, results were extracted from the reviewed eligible texts and papers.Results:The most common ethical dilemmas included lack of patientcentered and empathic care, discrimination, lack of attention to patient autonomy and familycentered care, irresponsibility, as well as conflicts between professional commitment and personal responsibilities. All of the challenges mentioned above may cause ethical harm, mentalemotional consequences, moral stress, lack of psychological adjustment, and disrespect. Therefore, nurses can promote their skills, innovation, and creativity while coping with such challenges. Selfcare strategies, structural supports, and professional interventions would mitigate the negative implications while increasing positive ones.Conclusion: Nurses may face many problems in making ethical decisions while caring for patients who suffer from COVID19. Therefore, some effective coping strategies must be designed to encounter these challenges and support nurses during the crisis.
