تبیین چالش های آموزش به بیمار از دیدگاه پرستاران : تحلیل محتوی کیفی

مقدمه: یکی از ابعاد مهم مراقبت ‌های پرستاری و از نقش ‌های کلیدی پرستار در ارائه خدمات بهداشتی درمانی، آموزش به بیمار است. علیرغم فواید زیاد آموزش به بیمار و هزینه ‌های ناچیز آن، اجرای این امر مهم در برخی از مراکز درمانی با چالش ‌هایی روبرو می‌ گردد. مطالعه حاضر با هدف تبیین چالش‌ های آموزش به بیمار از دیدگاه پرستاران انجام شده است.روش کار: تحقیق حاضر یک مطالعه کیفی با رویکرد تجزیه و تحلیل محتوای قراردادی بر روی پرستاران شاغل در بیمارستان ‌های شرق گیلان در سال 1397 انجام شد. روش نمونه ‌گیری، هدفمند و روش جمع آوری اطلاعات به صورت مصاحبه‌ های نیمه ساختارمند و دارای سوالات با پاسخ باز بوده و تا رسیدن به اشباع اطلاعاتی ادامه داشت. جهت انجام فرآیند تحلیل داده ‌ها از مراحل پیشنهادی گرانهیم و لاندمن ( granneheim and lundman ) بهره گرفته شد و درستی و استحکام تحقیق بر اساس معیارهای guba and lincoln صورت گرفت. یافته‌ها: نتایج حاصل از 21 مصاحبه صورت گرفته شامل چهار طبقه اصلی و هفده زیر طبقه شامل: موانع مدیریتی(غفلت آموزش، فاصله دست‌های همکار، پزشکان ناهمسو)، بستر نامناسب سازمانی (نیروی ناکافی پرستاری، در تنگنای زمان، حریم ناامن، تجهیزات ناکافی، مرز ناپیدای آموزشی)، موانع مربوط به بیمار/ همراه بیمار (ضعف دانش، اضطراب همراه، سد فرهنگی، اعتقاد متزلزل، همراه ناآگاه) و موانع پرسنلی درک شده (بی‌اعتمادی رابطه، انگیزه کمرنگ، زبان سخت، فهم دشوار) بود.نتیجه ‌گیری: نتایج مطالعه نشان داده است که چالش‌ های آموزش به بیمار متاثر از از عوامل مختلفی بوده، که از این میان موانع مدیریتی و پرسنلی محسوس‌ تر می باشد. لذا جهت بهبود وضعیت آموزش به بیمار، همسو سازی مدیران در این زمینه و افزایش سطح مهارت‌ های ارتباطی پرستاران، امری ضروری است.

exploration the challenges of patient education from nurses' viewpoints: qualitative content analysis

introduction:one of the important dimensions of nursing care and one of the key roles of nurses in providing health care services is patient education. despite the great benefits of patient educating and its low cost, the implementation of this important task in some medical centers faces challenges. the aim of this study was to exploration the challenges of patient education from the perspective of nurses.methods and materials: the present study was a qualitative study with a contractual content analysis approach on nurses working in east of guilan hospitals in 2019. the sampling method was purposeful and data gathering method were semistructured interviews with openended questions and continued to reach information saturation. to carry out the data analysis process, the proposed steps of granneheim and lundman were used and the accuracy and strength of the research was determined based on guba and lincoln criteria.results: the results of 21 interviews consisted of four main categories and 17 subcategories: management barriers (neglect of teaching, the distance between the hands of a colleague, uncoordinated physicians), inadequate organizational context (inadequate nursing staff, in time constraints, insecure privacy, insufficient equipment, invisible educational boundaries), patientrelated barriers/ patientchr('39')s companion (knowledge weakness, accompanying anxiety, cultural barriers, shaky beliefs, unconscious beliefs) and barriers of perceived personnel (distrust of relationship, diminutive motivation, hard language, difficult understanding).conclusion: the results of the study have shown that the challenges of patient education are affected by various factors, which managerial and personnel barriers are more noticeable. therefore, in order to improve the situation of patient education, it is necessary to align managers in this field and increase the level of communication skills of nurses.
