بررسی تاثیر آموزش اصول اخلاق حرفه‌ای در نگرش مدیران بالینی پرستاری به فرهنگ ایمنی بیمار

مقدمه: فرهنگ ایمنی بیمار از مهم ‌ترین جنبه کیفیت مراقبت ‌های بالینی است. برآیند مطالعات رعایت ایمنی بیمار را در مراکز درمانی نامطلوب نشان می‌ دهد. شناخت روش‌ های اثر بخش در ارتقا این مولفه از اولویت ‌های سازمانی است. هدف این مطالعه بررسی تاثیر آموزش اصول اخلاق حرفه ‌ای در نگرش مدیران بالینی پرستاری به فرهنگ ایمنی بیمار بود.روش کار: مطالعه نیمه تجربی حاضر به‌ صورت پیش ‌آزمون و پس ‌آزمون در سال 1395 انجام شد. نمونه پژوهش 30 پرستار رده مدیریت بالینی شاغل در مرکز درمانی شهید صیاد شیرازی گرگان بود. روش نمونه ‌گیری به‌ صورت در دسترس با تصادف سازی در هر گروه انجام شد. پرستاران در دو گروه تجربی (آموزش اصول اخلاق حرفه‌ ای با رویکرد فرهنگ ایمنی بیمار) و شاهد (بدون هیچ‌ گونه مداخله) تقسیم شدند. آموزش ‌ها در 8 دوره پروتکل آموزشی هر یک به مدت 86 ساعت به فاصله زمانی هر 5 تا 7 روز به شیوه کارگاه ارائه شد. ابزار مطالعه شامل اطلاعات دموگرافیک و مقیاس استاندارد پیمایش بیمارستان از نظر فرهنگ ایمنی بیمار (hsops) بود. داده‌ ها با استفاده از نرم‌افزار spss نسخه‌ی 16، آمار توصیفی و آزمون‌ های کولموگروف اسمیرنوف، کای اسکوئر، تی مستقل و تی زوجی در سطح معنی ‌داری 0.05>p تجزیه ‌و تحلیل شد.یافته‌ ها: تعداد 17 نفر (56.7 درصد) از شرکت‌ کنندگان زن بودند. میانگین سنی پرستاران 6.7 ±39.5 سال بود. قبل از شروع کارگاه میانگین فرهنگ ایمنی مددجو در گروه مداخله 5.3 ±50.6 و گروه شاهد 5.9 ±48.9 بود. پس از ارائه آموزش‌ ها اختلاف معناداری (0.012=p) در میانگین نمره گروه تجربی (13.7 ±139.2) نسبت به گروه شاهد (3.09 ±53.5) ایجاد شد. نتایج آزمون تی زوجی در گروه مداخله نشان داد میانگین فرهنگ ایمنی بعد از انجام مداخله (13.7 ±139.2) به‌صورت معنا داری (21.092=t، 0.0001=p) نسبت به قبل از مداخله (5.3 ±50.6) افزایش داشت درحالی ‌که در گروه کنترل اختلاف معناداری (1.970=t، 0.1=p) نداشت.نتیجه ‌گیری: مطالعه حاضر نشان داد آموزش اصول اخلاق حرفه ‌ای مراقبتی در ارتقاء نگرش مدیران بالینی پرستاری به فرهنگ ایمنی بیمار موثر بود. لذا مداخله با تاکید بر آموزش اصول اخلاقی به‌عنوان روشی موثر در برنامه‌ های آموزشی ارتقای فرهنگ ایمنی بیمار پرستاران توصیه می‌ گردد.

survey of effectiveness, professional ethics training in the attitude of clinical management nurses to the patient safety culture

introduction:the patient safety culture is one of the most important aspects of the quality of clinical care. the results of the study showed that patient safety was observed in undesirable treatment centers. understanding effective methods in promoting this component is an organizational priority. the aim of this study was to survey of effectiveness, professional ethics training in the attitude of clinical management nurses to the patient safety culture.methods materials: quasiexperimental study was performed using pretest and posttest and control group in 2016. the sample consisted of 30 clinical management nurses working in shahid sayyad shirazi hospital in gorgan. sampling method was available as a randomization with each group. the nurses were divided into two experimental groups (training of professional ethics of care with a patient safety culture approach) and control (without any intervention). training was offered in 8 of the protocol for 86 hours training each interval every 5 to 7 days through workshop. the study tool included demographic information and a standard hospital scanning scale in the patient safety culture (hsops). data were analyzed by spss software version 16, descriptive statistics, and kolmogrovsmirnov, chisquare, independent ttest and paired ttest at a significant level of p <0.05.results: total of participants were 17 female (56.7%). the mean age of the nurses was 39.5 ± 6.7 years. before the start of the workshop, the mean safety culture of the patient in the intervention group was 50.6 ± 3.5 and the control group was 48.9 ± 5.9. after providing the training, a significant difference (p=0.012) in the mean score of the experimental group (139.2 ± 13.7) was compared to the control group (53.5 ±3.09). the results of paired ttest were shown in the intervention group the mean safety culture after the intervention increased (139.2 ± 13.7) significantly (t=21.29, p=0.0001) compared to the preintervention (50.6 ±3.5) however, there was no significant difference in the control group (t=0.970, p=0.1).conclusion: this study showed career ethics training was effective in enhancing nurseschr('39') attitudes towards levels to patient safety culture. therefore, it is recommended that intervention with an emphasis on teaching ethics as an effective method in educational programs promoting nurseschr('39') safety culture.
