بررسی میزان خطاهای انسانی در فرآیند پذیرش بیماران در اتاق عمل در بیمارستان‌های منتخب دانشگاه علوم پزشکی اصفهان (1396)

مقدمه: جراحی، نقش عمده ای در مراکز درمانی در دنیا ایفا می کند. بیمارانی که تحت عمل جراحی قرار می گیرند به مراقبت پرسنل اتاق عمل نیازمند هستند. اما پرسنل هر اندازه با تجربه، متعهد و بادقت باشند، ممکن است در انجام وظایف حرفه‌ای خود دچار خطا شوند. آشکارسازی خطاها به عنوان پایه و اساس حفظ و ارتقای ایمنی بیماران شناخته شده است. یکی از زمان‌های حساس و پرمخاطره از نظر بروز خطا، پذیرش بیمار جراحی می باشد. از این رو پژوهش حاضر با هدف بررسی خطا در فرآیند پذیرش بیمار در اتاق عمل انجام گردید.روش کار: این پژوهش، تحقیقی توصیفی-مقطعی، که در اتاق عمل بیمارستان‌های منتخب دانشگاه علوم پزشکی اصفهان (1396 ) انجام گرفت. ابزار پژوهش پرسشنامه محقق ساخته، شامل: اطلاعات دموگرافیک و 11 سوال در مورد میزان خطا در فرآیند پذیرش بود. داده‌ها با نرم افزار spss-21 و آزمون‌های آمار توصیفی مورد تجزیه و تحلیل قرار گرفت.یافته‌ها: به طور کلی 212 نفر پرسشنامه‌ها را به طور کامل تکمیل کردند که میانگین درصد خطا در فرآیند پذیرش در اتاق عمل 29.4 درصد بود به ترتیب مواردی از جمله زمان ناشتا بودن بیمار (40.2 درصد) ارتباط صحیح با بیمار (39.7 درصد)، دارای بیشترین درصد خطا و کم ترین میزان خطا مربوط به چک محل و سمت عمل جراحی (11.5 درصد) بود. نتیجه‌گیری: با توجه به بروز خطا در فرآیند پذیرش بیمار در اتاق عمل‌ها، که می‌تواند زمینه‌ساز مشکلات و خطراتی گردد، پرسنل و مسولین اتاق عمل باید تدابیر لازم را جهت ریشه یابی و کاهش میزان این خطاها به کار بندند.

Evaluation of Human Errors in Patient Admission Procedure in Operative Room in Selected Hospitals of Isfahan University of Medical Sciences in 2017-2018

Introduction:Surgery has played a major role in medical centers in the world. The operating room staff are required to take care of patients undergoing surgery. However, staffed with diligence and commitment, are likely to make mistakes in their professional tasks. Detection of errors and mistakes is recognized as the basis for maintaining and improving patient safety. One of the most sensitive and hazardous times in operating room is admitted of the patients. Therefore, the present study was aimed at investigating the errors in the patient admission process in the operating room.Methods and Materials: This descriptivesectional study was conducted in the operating rooms of selected hospitals of Isfahan University of Medical Sciences in 20182019. The research instrument was a researchermade questionnaire including demographic information and 11 questions concerning errors in the patientchr('39')s admission process. Data were entered into SPSS21 and analyzed by descriptive statistical tests.Result:212 people completed the questionnaire and the mean error in the patient admission process in the operating room was 29.39%. The highest scores were related to the patientchr('39')s fasting time by 40.2% and correct relationship with the patient during the admission by 39.7%, and check the site of surgery had lowest score (11.5%).Conclusion: Given the incidence of errors in the patient admission process in the operating room, which can pose problems and risks, operating room personnel and personnel should take the necessary measures to eliminate and reduce these errors.
