قصور در حفظ ایمنی بیمار دچار ترومای دست بر اساس گزارشی اخلاقی

مقدمه: به خطر انداختن ایمنی بیمار ، با تاخیر در ارائه مراقبت توسط اعضای تیم درمان می تواند آسیب های جبران ناپذیری برای بیمار به دنبال داشته باشد. در این پژوهش یک مورد چالش اخلاقی ناشی از تاخیر در مراقبت بیمار معرفی گردیده است.معرفی بیمار: مورد بررسی، آقایی 44 ساله با ترومای نافذ ناشی از فرز برقی دچار آسیب عصب و تاندون انگشتان دست با خونریزی فراوان بوده که به بخش اورژانس مراجعه کرده است. خدمات انجام شده، صرفاً زخم پانسمان و بدون تزریق واکسن کزار و آنتی ‌بیوتیک و ارائه آموزش های لازم بوده است. مددجو بعد از 24 ساعت به صورت شخصی به بیمارستانی در تهران ارجاع داده می شود. به دلیل ازدحام مراجعین بعد از 42 ساعت ناشتا بودن تحت عمل جراحی قرار می‌گیرد و در نتیجه دچار عوارض تاخیر در جراحی عصب و تاندون می شود.یافته‌ ها: در این گزارش، سلامتی و ایمنی بیمار به خطر افتاده است. با قصور تاخیر در درمان به دلیل نداشتن ویزیت به موقع، بیمار توسط پزشکان متخصص آنکال، عدم اجرای دستورات دارویی پزشک (آنتی بیوتیک و واکسن کزاز) با وجود دریافت هزینه از بیمار، انتقال ندادن اطلاعات و آموزش کافی جهت حمایت بیمار در تصمیم گیری برای اخذ رضایت شخصی و ترک بیمارستان، بروز عوارض تاخیر در جراحی و ناشتا بودن بیش از حد نیاز بیمار.نتیجه‌ گیری: قصور در حفظ ایمنی بیمار، عواقب جبران ناپذیری برای بیماران و نظام ارائه خدمات در پی دارد. در نتیجه راه حل های مناسبی در جهت برنامه ریزی دوره های آموزشی ضمن خدمت توسط مسئولین جهت افزایش دانش حرفه ای، تعهد، مسئولیت پذیری، پاسخگویی، حساسیت اخلاقی و شناخت چالش های اخلاقی توسط کادر درمان در جهت پیشگیری از قصور تاخیر در درمان بیماران است.

Malpractices in the Patient Safety Suffered from Hand Trauma: An Ethical Case Report

Introduction:Endangering patient safety by delaying the care provided by members of the treatment team can cause irrecoverable harm to the patient. In this study, a case has been presented with the ethical challenge of delay in patient care.Case Report:The patient was a middleaged man suffering from Penetrating trauma to the nerve and tendon of the fingers hand with extensive bleeding by electric milling. He referred to the emergency department. The only Services performed for him included; wound dressing, without inject tetanus vaccine and antibiotics and providing necessary training. The client was referred personally to a hospital in Tehran after 24 hours. Eventually, the patient underwent surgery after 42 hours of fasting due to overcrowding of patients. As a result, the patient involved in complications due to delayed nerve and tendon surgery.Results:In this report, patient safety was compromised with delay in treatment. This risk was due to the lack on time visits by specialist physicians, delayed surgical complications and prolonged fasting, failure to comply with physician orders (antibiotics and tetanus vaccine) despite payment by the patient, lack of transfer information and training Sufficient to support the patient in making a personal decision to leave the hospital.Conclusion:Failure to maintain patient safety has irrecoverable consequences for patients and the service delivery system. As a result, the solution is to nurse managers; planning inservice education to the staff for increase professional knowledge, commitment, responsibility, accountability, ethical sensitivity and recognition of ethical challenges to prevent delays in patient treatment.
