تحلیل امکان‌پذیری تامین منابع پیاده‌سازی نقشه‌ی درمان کشور 1404

مقدمه: وزارت بهداشت و درمان ایران در سال 1395 اقدام به تدوین نقشه‌ی ‌راه درمان ایران 1404 نمود تا برنامه‌ریزی برای گسترش منابع راه‌بردی نظام سلامت به‌منظور دست‌یابی به حداقل‌های لازم، با دقت بیش‌تری صورت گیرد. در این نقشه به برآورد تعداد نیروی انسانی، تجهیزات پزشکی سرمایه‌ای، تخت‌های بیمارستانی و تسهیلات اورژانس لازم پرداخته شد. پژوهش حاضر برای برآورد هزینه‌های پیاده‌سازی این نقشه و بررسی امکان‌پذیر بودن تامین منابع لازم برای اجرای آن انجام شد.روش کار: با استفاده از هزینه تحقق هر یک از اهداف در نظر گرفته شده در سال 1395، به برآورد این هزینه‌ها در 10 سال آینده پرداخته شد و با روشی مشابه، منابع در دست‌رس بخش سلامت در همان سال برآورد گردید. با توجه به بالاتر بودن هزینه‌ها نسبت به منابع در دست‌رس، سناریوهای ممکن برای پیاده‌سازی نقشه ارایه شد.یافته‌ها: درسال 1404، بیش‌ترین هزینه‌های نقشه مربوط به نیروی‌انسانی (71%) و سپس ساخت و تجهیز تخت‌های بستری(20%) است و تامین تجهیزات، کم‌ترین سهم (1/1%) از هزینه های برآورد شده را به خود اختصاص خواهد داد. با فرض سناریوی پایه، یعنی نرخ تورم سالیانه معادل 10 الی 16درصد، افزایش حقوق سالانه 10 الی 14درصد، نرخ رشد اقتصادی سالانه 3 الی 5درصد و سهم بخش سلامت از gdp معادل 7 الی 8 درصد، منابع بخش بهداشت و درمان برای پیاده‌سازی 80 درصد از اهداف نقشه راه کافی خواهد بود.نتیجه‌گیری: به‌منظور اجرای کامل نقشه راه درمان 1404 و با توجه به تلاطمات اقتصادی پس از طراحی نقشه‌ی راه درمان، تجدید نظر در خصوص منابع بخش بهداشت و درمان یا کاهش هزینه‌های نقشه‌ی راه درمان، ضروری است. 

feasibility analysis of 2025 iran health roadmap implementation

introduction: the ministry of health and medical education of iran in 2016 established a 2025 road map for the treatment sector to more carefully plan for the development of strategic health resources to achieve essential standards. this road map estimated the required human capital, health care facility, hospital beds, and emergency facility. the current study aimed to assess the costs of implementing the road map and its feasibility for the health care system.methods: we used the 2015 costs of achieving each goal in the road map to estimate the costs in the next 10 years. similarly, we calculated the accessible resources of the health system in the same year. we proposed different possible scenarios to implement the road map based on the costs being higher than resources.results: in 2025, the highest cost of the road map will go to human capital (71%), followed by providing equipped hospital beds (20%), while providing equipment will have the least share of costs (1.1%). according to the basic scenario, considering the 10 to 16% annual inflation rate, 10 to 14% increased rate of wages, 3 to 5% economic growth, and 7 to 8% gdp share of health system expenditure, the health system can implement 80% of the road map in 2015.conclusion: considering the iran economic turmoil after finalizing the road map, it is necessary to revise the resources of the health care system and reduce the costs of the road map for the complete implementation of the map.keywords: road map; cost; estimation; simulation; iran please cite this article as follows:sabermahani a, noori hekmat s, haghdoost aa. feasibility analysis of 2025 iran health roadmap implementation. hakim health sys res. 2019; 22(3): 230-240. *corresponding author: professor, ph.d. in epidemiology and biostatistics, social determinants of health research center, future studies research center in health, kerman university of medical sciences, kerman, iran. tel: +98-3431325402, email: ahaghdoost@gmail.com
