تبیین و پیش‌بینی رفتار مشارکت خیرین در نظام سلامت ایران

مقدمه: خیرین سلامت در تامین مالی، تولید منابع و ارایه‌ی خدمات سلامت مشارکت دارند. عوامل مختلفی بر رفتار خیرخواهانه‌ی خیرین سلامت اثر می‌گذارند. هدف پژوهش حاضر، تبیین و پیش‌بینی رفتار مشارکت خیرین در نظام سلامت ایران است.روش کار: مطالعه‌ی استقرایی حاضر، به‌صورت کیفی با ‌استفاده ‌از مصاحبه‌ی نیمه‌ساختار‌‌یافته با 38 نفر از خیرین سلامت و 26 نفر از سیاست‌گذاران، مدیران و کارشناسان حوزه‌ی معاونت اجتماعی وزارت بهداشت و دانشگاه‌های علوم پزشکی کشور انجام شد. مصاحبه‌ها به‌صورت هدف‌مند و به‌روش گلوله‌‌برفی تا مرحله‌ی اشباع داده‌ها ادامه یافت. برای تحلیل داده‌ها، از روش تحلیل موضوعی استفاده شد. کلیه‌ی ملاحظات اخلاقی در پژوهش حاضر رعایت شد.یافته‌ها: خیرین سلامت، رفتاری هدف‌مند و برنامه‌ریزی‌شده را دنبال می‌کنند. عوامل هدف، نیت، نگرش، هنجار، ویژگی‌های شخصی، دانش زمینه‌ای، رفتار گذشته، تجربه‌ی گذشته و کنترل رفتار بر رفتار مشارکت خیرین در تامین مالی، تولید منابع و ارایه‌ی خدمات سلامت موثر هستند. مشارکت خیرین در نظام سلامت، تابعی از هدف، نیت و نگرش آن‌ها نسبت‌ به مشارکت در حوزه‌ی سلامت و کنترل آن‌ها بر انجام این رفتار است. هنجارها، ویژگی‌های شخصی، دانش زمینه، تجربه‌ی گذشته و رفتار گذشته‌ی خیرین بر نگرش‌ آن‌ها نسبت‌به فعالیت‌های خیرخواهانه اثر می‌گذارد. همچنین، هنجارهای اخلاقی، توصیفی و ذهنی بر تقویت نگرش خیرین نسبت‌به مشارکت در بخش سلامت موثر هستند.نتیجه‌گیری: در مطالعه‌ی حاضر، مدلی برای پیش‌بینی و تبیین رفتار هدف‌مند و برنامه‌‌ریزی‌‌شده‌ی خیرین سلامت توسعه یافت. سیاستگذاران و مدیران نظام سلامت می‌توانند از این مدل برای تقویت مشارکت خیرین در تامین مالی، تولید منابع و ارایه‌ی خدمات سلامت استفاده کنند.

Several factors can influence the donors rsquo; contribution to the health system. This study aimed to explain and predict the donors rsquo; participation in the Iran health system.Methods: A qualitative and inductive study was performed using semistructured interviews with 38 donors and 26 policymakers and managers in the Social Affairs Department of the Ministry of Health and the Universities of Medical Sciences in 2018. The interviewees were recruited using a purposeful and snowball method. The thematic content analysis method was used for data analysis. All major ethical considerations were observed in this research.Results: Healthcare donors followed a purposeful and planned behavior for contributing to the health system. Nine factors can influence a donor rsquo;s charitable behavior in the health system, including goal, intention, attitude, norms (ethical, descriptive, and subjective norms), personal characteristics, background knowledge, experience, past behavior, and control of perceived behavior. The donors rsquo; participation in the health system is a function of their goals, intentions, attitudes, and their control of perceived behavior. The donors rsquo; attitude is shaped by their characters, background knowledge, experience, past behavior, and ethical, descriptive, and subjective norms.Conclusion: The purposeful and planned behavior model developed in this study guides healthcare managers and policymakers in explaining and predicting the donors rsquo; charitable behavior in the health system.Keywords: Purposeful and Planned Behavior Theory; Donors; Iranian Health System; ModellingPlease cite this article as follows:Mosadeghrad AM, Ehteshami F.
