درجه ی برخورداری و نحوه ی توزیع امکانات سلامت در شهرستان های استان اصفهان

مقدمه: بررسی وضعیت سلامت مناطق مختلف ایران، گامی مهم در جهت تخصیص بهینه ی منابع سلامت میان مناطق مختلف کشور است. هدف مطالعه ی حاضر، تعیین درجه ی برخورداری و میزان نابرابری در توزیع امکانات سلامت میان شهرستان های استان اصفهان طی برنامه پنجم توسعه بود.روش کار: براین اساس، نخست با روش تاپسیس، درجات برخورداری از امکانات سلامت محاسبه و سپس با بهره گیری از شاخص ضریب جینی، نحوه ی توزیع آن ها میان 23 شهرستان استان اصفهان طی سال های 941390 (برنامه ی پنجم توسعه) تعیین گردید. داده های این پژوهش از سال نامه های آماری استان در بازه ی زمانی تحت مطالعه استخراج گردید.یافته ها: نتایج مطالعه ی حاضر نشان داد که شهرستان های استان اصفهان درجه ی برخوراری یک سانی ندارند. روند کلی ضریب جینی برخورداری از امکانات سلامت طی برنامه ی پنجم توسعه سیر صعودی داشته است. بر اساس ضرایب جینی محاسباتی، نابرابری در توزیع امکانات سلامت در سال پایانی نسبت به سال ابتدایی برنامه، تقریبا 38درصد افزایش داشته؛ به طوری که در سال ابتدایی برنامه، ضریب جینی برابر 255/0 بوده و در سال 1394 به 353/0 تغییر یافته است.نتیجه گیری: افزایش نابرابری در توزیع امکانات سلامت، به نفع شهرستان های بیشتر برخوردار و به ضرر شهرستان های کمتر برخوردار، حاکی از آن است که رفاه شهرستان های کمتر برخوردار در مقایسه با شهرستان های بیشتر برخوردار طی برنامه ی پنجم توسعه کاهش یافته است. با توجه به تشدید نابرابری، به ویژه در سال پایانی برنامه، جبران این نابرابری ها می تواند در دستور کار برنامه ریزان سلامت استان قرار گیرد.

This study tried to determine the enjoyment level and equality in the distribution of health facilities in the counties of Isfahan province during the 5th development plan of Iran.Methods: First, the enjoyment level of Isfahan counties was determined using the TOPSIS method. Then, the distribution of health facilities was evaluated for 23 counties during 2011 2015. The data used in this study were obtained from the statistical yearbook of Isfahan.Results: The counties of Isfahan province were not homogenous in terms of enjoyment level. The overall trend of the Gini coefficient was upward during the study period. The inequality in the distribution of health facilities increased by 38% from the beginning year of the plan to the last year of the study period so that the Gini coefficient was 0.255 in the first year of the plan and 0.353 in 2015.Conclusions: The increased inequality in the distribution of health facilities indicated that the welfare of lessfavored counties decreased compared to more developed counties during the fifth development plan.
