چالش‌ها و ملزومات دستیابی به عدالت در سلامت در جمهوری اسلامی ایران

مقدمه: از بین بردن بی عدالتی در سلامت، یکی از اهداف مهم نظام های سلامت است. هدف از طرح حاضر، بررسی دستاوردها، چالش ها و راهکارهای دستیابی به عدالت در سلامت در جمهوری اسلامی ایران بود.روش: این طرح با رویکرد کیفی انجام گرفت. اطلاعات با استفاده از مرور منابع و اسناد و بحث گروهی متمرکز با ذینفعان گردآوری گردید. اطلاعات پس از تحلیل، در سه دسته دستاوردها، چالش ها و راهکارها دسته بندی شد.نتایج: نظام سلامت کشور در زمینه کاهش نابرابری های سلامت در 10 سال گذشته دستاوردهای فراوانی داشته که مهم ترین آن ها عبارتند از توسعه شبکه های بهداشتی به حاشیه شهرها، طرح پزشک خانواده (شهری و روستایی)، طرح تحول سلامت و در پی آن کاهش هزینه های کمرشکن و فقرزای سلامت، افزایش دسترسی عموم به اطلاعات سلامت و به دنبال آن ایجاد پاسخ گویی در سیاست گذاران. عواملی نیز موجب شده که دستاوردهای بیشتری حاصل نشود که اجرا نشدن یا اجرای ناکامل قوانین و سیاست های اجرایی، توجه ناکافی به مولفه های اجتماعی سلامت و ضعف در تدوین و اجرای پیوست سلامت برای سیاست ها از آن جمله هستند.نتیجه گیری: به نظر می رسد در تدوین برنامه و قانون گذاری مشکل زیادی در کشور وجود نداشته باشد و قوانین تقریبا همسو با توصیه های سازمان جهانی بهداشت، در راستای کاهش نابرابری ها در سلامت باشند. اما میان سیاست گذاری و تدوین قوانین با اجرای آن ها انفکاک وجود دارد؛ به عبارت دیگر، هنگام نوشتن قوانین به امکان اجرا اندیشیده نمی شود و اصول حکم رانی خوب، از جمله جلب همکاری و مشارکت ذینفعان، کمتر رعایت می شود.

The aim of this study was to assess the achievements and challenges, as well as solutions, in reaching the equity in health in Iran.Methods: This study was conducted using a qualitative approach. Data were gathered by reviewing documents and conducting focused group discussions with the stakeholders. The analyzed data were categorized into three groups including achievements, challenges, and solutions.Results: The Iranian health system has achieved many successes in the past 10 years including expanding the primary healthcare networks to city outskirts, implementing the family physician program (urban and rural), implementing the Health Transformation Plan (HTP) and subsequent decrease in catastrophic and impoverishing health expenditures, and increasing the public access to health information and subsequent responsiveness of policymakers. However, some factors hindered further achievements, including the absent or incomplete implementation of laws and practical policies, insufficient attention to the social determinants of health, and poor designing and implementation of the health annex for the policies.Conclusions: It seems that there are little problems in legislation and policymaking in the country and the laws are almost compatible with the recommendations of the World Health Organization, aiming at the inequity reduction in health. However, there is a gap between the policymaking/legislation and the practice.
