سنجش عدالت در تامین مالی هزینه‌های سلامت خانوارها در برنامه‌های توسعه‌ی ایران

مقدمه: مشارکت ناعادلانه مالی مردم و پرداخت های مستقیم از جیب که متناسب با توان پرداخت آن ها نباشد، نه تنها موجب می شود خانوارهای بیشتری در گروه مواجه با هزینه های کمرشکن سلامتی قرار گیرند، بلکه ممکن است برخی خانوارها به خاطر ناتوانی در پرداخت این هزینه ها به زیر خط فقر سقوط کنند. این مطالعه با هدف بررسی دقیق وضعیت عدالت و برابری در تامین مالی هزینه های سلامت در سطح کل کشور و همچنین به تفکیک پنج بیستک هزینه ای انجام گرفته است.روش کار: در این مطالعه براساس رویکرد بانک جهانی با استفاده از شاخص عدالت در مشارکت مالی خانوارها، شاخص درصد خانوارهای مواجه با هزینه های کمرشکن و همچنین شاخص درصد هزینه های فقرزا، عدالت در تامین مالی هزینه های سلامت طی سال های منتخب بررسی شد. به همین منظور، از داده های پیمایش هزینه درآمد خانوارهای (شهری) طی سال های 13941363 مرکز آمار ایران استفاده شد.یافته ها: شاخص عدالت در مشارکت مالی خانوارها طی سال های منتخب نوساناتی جزیی داشته است. درصد خانوارهای مواجه با هزینه های کمرشکن افزایش اما درصد خانوارهای مواجه با هزینه های فقرزا طی سال های منتخب کاهش یافت. بیستک پنجم هزینه ای شاهد بیش ترین درصد خانوارهای مواجه با هزینه های کمرشکن و کمترین میزان عدالت و بیستک اول شاهد بیش ترین میزان مواجه با هزینه های فقرزا بود.نتیجه گیری: روند افزایشی درصد خانوارهای مواجه با هزینه های کمرشکن و همچنین افزایش بی عدالتی در مشارکت مالی خانوارها نشان داد که برنامه های اجرا شده طی سال های منتخب به حد کافی موثر نبوده و مشارکت مالی در پرداخت های سلامتی به خوبی هدفمند نشده است، درنتیجه نظام سلامت نتوانسته است آن چنان که باید در راستای حفاظت از خانوارها در برابر هزینه های کمرشکن عمل کند.

Therefore the main goal of this essay is to analyze the situation of fairness and equality infinancial contribution of health payments. These calculations have been done throughout the whole country.In addition, the household’s expenditures in Iran are disaggregated into expenditure quintiles through selectedyears.Methods: This study aimed to investigate health financing by using FFC index, catastrophic healthexpenditures index, and impoverishment expenditures index (based on World Bank Approach). To this end,Iran’s annual household’s survey data, which is produced by Iran statistic center, is used (1989 2014).Results: The index of fairness in financial contribution of households during the selected years experiencedminor fluctuations. The percentage of the households facing the catastrophic costs increased, however, thepercentage of impoverishment expenditures decreased in these selected years. Among the quintiles, thehouseholds in the fifth quintile had the highest rate of inequality in health payments and the highest percentageof households facing catastrophic costs. The first quintile had the highest rate of impoverishment expenditures. Conclusions: It is concluded that the performed programs during the selected years have not been effective enough and financial contributions in health payments has been poorly targeted.
