بررسی ارزش دندان‌پزشکی از راه دور برای تشخیص پوسیدگی‌های دندانی با استفاده از روش dmft در کودکان 12‌ساله شهر قم

زمینه و هدف: یکی از مفاهیم وارد‌شده به حوزه دندان‌پزشکی که باعث سهولت کار برای دندان‌پزشکان و مددجویان این حیطه شده است، تله‌دنتیستری است. در بسیاری از کشورها از جمله ایران، منابع مالی مورد‌نیاز برای مجهز کردن شهرها به بیمارستان‌ها یا درمانگاه‌های دندان‌پزشکی کافی نیستند و از سوی دیگر، نیروی انسانی لازم از جمله دندان‌پزشک، منشی و غیره برای حضور و فعالیت در این مراکز در دسترس نیستند. در این مطالعه فواید و مزایای استفاده از این تکنولوژی نوین را در شهر قم بررسی کردیم.روش بررسی: این پژوهش یک بررسی ارزش تشخیصی است که با طراحی یک مطالعه مقطعی در سال 1398 انجام شد. این پژوهش به کمک یک دندان‌پزشک و یک دستیار انجام شد. بدین‌صورت که در مرحله اول، دندان‌پزشک، دهان بیمار را معاینه کرد و دستیار، پس از آموزش‌دیدن جهت عکس‌برداری اصولی، از دهان و دندان شرکت‌کننده عکس‌برداری کرد. سپس عکس تهیه‌شده بعد از دو هفته به دندان‌پزشک نشان داده شد و دندان‌پزشک نظر خود را بیان کرد. مقایسه دو نظر اعمال‌شده از طریق شاخص dmft انجام شد. از آمار توصیفی برای گزارش میانگین و انحراف معیار و فراوانی داده‌ها و از نرم‌افزار آماری spss برای تعیین حساسیت و ویژگی و صحت تشخیص پوسیدگی توسط روش فتوگرافی در مقابل تشخیص حضوری استفاده شد.یافته‌ها: 147 نفر به صورت آگاهانه و داوطلبانه در این مطالعه شرکت کردند که بررسی بر روی کل دندان‌های آن‌ها صورت پذیرفت. مقدار حساسیت روش dmft در تشخیص دندان‌های سالم مثبت و بالا و در حدود 79 درصد است. همچنین ویژگی روش تله‌دنتیستری برای تشخیص دندان‌های پوسیده در حدود 94 درصد است که درصد قابل توجهی است.نتیجه‌گیری: بر اساس این داده‌ها می‌توان این نتیجه را گرفت که زین پس با توجه به پیشرفت تکنولوژی و پیشرفت ارتباطات زیرساخت در سطح کشور و به‌خصوص استان و شهر قم و با توجه به بُعد مسافت در کشور ایران می‌توان از دندان‌پزشکی از راه دور به عنوان روشی نوین در غربالگری و تشخیص و طرح درمان بیماری‌های دهان و دندان استفاده کردکه خود باعث کاهش هزینه‌های بیمار به‌خصوص هزینه‌های پنهان آن (از جمله اتلاف وقت) می‌شود.

Diagnostic Value of Tele-dentistry in Decay Detection by DMFT Method

Background and Objectives: Teledentistry is among the concepts introduced into the dental field that facilitated the efforts of dentists and hygienists in this field. In numerous countries, including Iran, the financial resources and equipment for cities with hospitals or dental clinics are insufficient. Furthermore, the necessary human resources, such as dentists, secretaries, etc. are unavailable to operate in these centers. This study explored the benefits of using this new technology.Methods: This was a diagnostic value study using a crosssectional design, i.e., conducted in Qom City, Iran, in 2019. This research was performed with the aid of a dentist and an assistant. Initially, the dentist examined the explored patient rsquo;s mouth, and the assistant, after being trained in the principle of photography, captured the research participant rsquo;s mouth. Then, the photo was illustrated to the dentist after two weeks and the dentist expressed his opinion. Comparing the two comments was established using the DMFT index. Descriptive statistics were used to report the mean and standard deviation and frequency of the collected data. Furthermore, SPSS was used to determine the sensitivity, specificity, and accuracy of caries diagnosis by photographic versus face detection method.Results: The sensitivity of the DMFT method in detecting healthy teeth was positive and high, i.e., approximately 79.0%. Moreover, the feature of the telemedicine method for the detection of decayed teeth was calculated to be about 94.0%, i.e., a significant percentage.Conclusion: Based on the obtained data, due to the advances in technology and the development of infrastructure communications in the country, especially in Qom Province and in Iran, given the distance dimension in Iran, telecommunication dentistry can be a novel method used in the screening, diagnosis, and treatment of oral diseases. Accordingly, it could help reduce the costs imposed on the patient, especially its latent costs (including the waste of time).
