مقایسه شاخص‌های عملکردی اورژانس بیمارستان شهید بهشتی قم، قبل و بعد از طرح تحول سلامت

زمینه و هدف: با توجه به شروع طرح تحول سلامت وزارت بهداشت، درمان و آموزش پزشکی در سال 1393، ارزیابی این طرح از اهمیت ویژه ای برخوردار است. این مطالعه با هدف بررسی شاخص های عملکردی اورژانس بیمارستان شهید بهشتی قم (قبل و بعد از طرح تحول) انجام شد.روش بررسی: این مطالعه به روش توصیفی مقطعی در بیمارستان شهید بهشتی قم (سال 1393) صورت گرفت. جامعه آماری شامل اطلاعات بیماران مراجعه کننده به اورژانس بیمارستان بود که از پرونده های اورژانس و بایگانی، جمع آوری و در بانک اطلاعاتی وارد گردید. چهار مورد از شاخص های عملکردی بخش اورژانس، انتخاب و قبل و بعد از طرح تحول سلامت مورد ارزیابی قرار گرفت. این شاخص ها شامل: درصد بیماران تعیین تکلیف زیر 6 ساعت، درصد میزان بستری بیماران زیر 12 ساعت، میزان احیای ناموفق و ترک با رضایت شخصی بود. داده ها با استفاده از آزمون کای اسکوئر تحلیل شدند.یافته ها: در قبل و بعد از طرح تحول سلامت، شاخص درصد تعیین تکلیف بیماران زیر 6 ساعت از 86% به 78.5%، درصد میزان بستری بیماران زیر 12 ساعت از 96% به 83%، ترک با رضایت شخصی بیماران از 19% به 14% و شاخص درصد احیای ناموفق نیز از 83% به 55.3% رسید. میانگین این چهار شاخص در 4 سال بعد از اجرای طرح تحول نسبت به قبل از آن، ازنظر آماری تفاوت معنی داری داشت. p<0.0001.نتیجه گیری: نتایج این مطالعه نشان داد شاخص های عملکردی اورژانس با اجرای طرح تحول بخش سلامت بهبود یافته است؛ بنابراین لازم است این مطالعه در سطح وسیع تری برای کل شاخص های بیمارستانی انجام گیرد.

Comparison of the Performance Indicators of the Emergency Department of Qom Shahid Beheshti Hospital before and after the Health Transformation Plan, (Iran)

Background and Objectives: Considering the initiation of the Health Transformation Plan in the Ministry of Health and Medical Education in 2014, evaluation of this plan is of particular importance. The aim of this study was to investigate the performance indicators of the Emergency Department of Shahid Beheshti Hospital in Qom city (before and after the transformation plan). Methods: The study was carried out as a descriptive crosssectional study in Shahid Beheshti Hospital of Qom city (year 2014). The statistical population consisted of the patients referring to the Emergency Department of the hospital, which was selected from the emergency and archived records and entered into the database. Four emergency department performance indicators, were selected and evaluated before and after the Health Development Plan. These indicators included the percentages of patients who were disposed under 6 hours, percentage of hospitalization of patients under 12 hours, unsuccessful cardiopulmonary resuscitation (CPR), and discharge with personal satisfaction. Data analysis was performed using the Chi square test. Results: Before and after the Health Transformation Plan, the percentage of patients who were disposed under 6 hours, reached from 86% to 78.5%, hospitalization rate of patients under 12 hours reached from 96% to 83%, discharge with personal satisfaction from 19% to 14%, and the percentage of unsuccessful cardiopulmonary resuscitation from 83% to 55.3%. The mean of these four indicators, was statistically significant 4 years after the implementation of the transformation plan (p<0.0001). Conclusion: According to the results of this study, emergency performance indicators improved with the implementation of the Health Transformation Plan. Therefore, this study should be carried out at a broader level for all hospital indicators.
