   تاثیر جراحی ارتوپدی در سینماتیک راه رفتن، تعادل وضعیتی و سرعت راه رفتن در کودکان مبتلا به فلج مغزی اسپاستیک: رویکردی چندبعدی

چکیده    مقدمه: فلج مغزی اسپاستیک (cp) شایع‌ترین ناتوانی حرکتی در کودکان است که با اسپاستیسیتی مزمن، سفتی عضلانی و اختلال در کنترل حرکتی مشخص می‌شود و به اختلالات راه رفتن، ناپایداری وضعیتی و کاهش سرعت راه رفتن منجر می‌شود. جراحی ارتوپدی شامل طولانی کردن تاندون‌ها و رهاسازی عضلات به‌طور روزانه برای اصلاح ناهنجاری‌های اسکلتی-عضلانی و بهبود عملکرد حرکتی به کار می‌رود. بااین‌حال، میزان تاثیر این جراحی‌ها در سینماتیک راه رفتن، تعادل وضعیتی و سرعت راه رفتن نامشخص است. هدف این مطالعه ارزیابی تاثیر جراحی ارتوپدی در سینماتیک راه رفتن، تعادل وضعیتی و سرعت گام‌برداری در کودکان مبتلا به cp اسپاستیک با استفاده از رویکردی چندبعدی بود.مواد و روش‌ها: مشاهده‌ای آینده‌نگرانه کوهورت بر روی 30 کودک (5 تا 12 سال، سطوح gmfcs i تا gmfcs iii)، که برای جراحی ارتوپدی برنامه‌ریزی شده بودند، انجام شد. شرکت‌کنندگان قبل از عمل و در فواصل 6 و 12 ماه پس از جراحی ارزیابی شدند. متغیرهای مورد بررسی شامل سینماتیک راه رفتن (طول گام، کادنس، زوایای مفصلی)، تعادل وضعیتی (مقیاس تعادل کودکان [pbs]، آزمون برخاستن و رفتن زمان‌دار [tug]) و سرعت راه رفتن (آزمون راه رفتن 10 متری [10mwt]) بودند. داده‌ها با استفاده از anova با اندازه‌های تکراری تحلیل شدند و اندازه اثر کوهن (cohen's d) محاسبه شد.نتایج و بحث: در 12 ماه پس از جراحی، بهبود معناداری در تمامی متغیرهای پیامدی مشاهده شد. طول گام 12/5 درصد (0/01>p) و کادنس 8/3 درصد (0/05>p) افزایش یافت. زاویه فلکسیون زانو و دُرسی‌فلکسیون مچ پا نیز به‌طور معناداری بهبود یافتند (0/01>p). تعادل وضعیتی بهتر شد، به‌طوری که نمره pbs از 42/3 به 48/7 افزایش (0/01>p) و زمان آزمون tug از 12/5 به 9/8 ثانیه کاهش یافت (0/01>p). سرعت راه رفتن در حالت عادی 18/6 درصد و در حالت سریع 2/15 درصد بهبود یافت (0/01>p). تحلیل زیرگروه‌ها نشان داد که کودکان با سطح gmfcs i بهبود بیشتری از سطوح gmfcs ii و gmfcs iii داشتند (0/05>p). تفاوت معناداری بین روش‌های طولانی کردن تاندون و رهاسازی عضلات مشاهده نشد (0/05>p).نتیجه‌گیری: جراحی ارتوپدی به‌طور معناداری سینماتیک راه رفتن، تعادل وضعیتی و سرعت گام‌برداری را در کودکان مبتلا به cp اسپاستیک بهبود می‌بخشد و این پیامدها تا 12 ماه پس از جراحی پایدار می‌مانند. یافته‌ها بر اهمیت مداخلات جراحی و پروتکل‌های توانبخشی فردی در بهبود تحرک عملکردی و کیفیت زندگی این بیماران تاکید می‌کنند.

   the impact of orthopedic surgery on gait kinematics, postural balance, and walking speed in children with spastic cerebral palsy: a multidimensional approach

Abstract    abstractintroduction: spastic cerebral palsy (cp) is the most prevalent motor disability in children, defined by chronic spasticity, muscle stiffness, and motor control impairment, resulting in gait disturbances, postural instability, and decreased walking speed. orthopedic surgery, including tendon lengthening and muscle releases, is routinely employed to rectify musculoskeletal deformities and enhance motor function. the extent to which these surgeries affect gait kinematics, postural balance, and walking speed is uncertain. the purpose of the current study was to assess the influence of orthopedic surgery on gait kinematics, postural balance, and gait speed in children with spastic cp via a multi-dimensional approach.materials & methods: a prospective observational cohort study was carried out in 30 children (5–12 years, gmfcs levels i–iii) who were scheduled for orthopedic surgery. participants were evaluated preoperatively and 6 and 12 months after surgery. outcomes were gait kinematics (stride length, cadence, joint angles), postural balance (pediatric balance scale [pbs], timed up and go [tug] test), and gait speed (10-meter walk test [10mwt]). data were analyzed using repeated-measures anova, and effect sizes (cohen's d) were determined.results & discussion: at 12 months after surgery, there were significant improvements in all outcome measures. stride length improved by 12.5% (p < 0.01), and cadence by 8.3% (p < 0.05). knee flexion angle and ankle dorsiflexion angle also improved significantly (p < 0.01). postural balance was improved, with pbs scores rising from 42.3 to 48.7 (p < 0.01) and tug test times falling from 12.5 to 9.8 seconds (p < 0.01). walking speed was enhanced by 18.6% in comfortable speed and 15.2% in fast speed (p < 0.01). subgroup analysis showed greater improvement in children with gmfcs level i compared to levels ii and iii (p < 0.05). no differences were noted between tendon lengthening and muscle releases (p > 0.05).conclusion: orthopedic surgery significantly improves gait kinematics, postural balance, and gait velocity in children with spastic cp, with the effects being preserved 12 months post-surgery. the findings emphasize the contribution of surgical interventions and personalized rehabilitation protocols in improving functional mobility and quality of life in these patients.
