موانع و چالش های کار تیمی در مراقبت تسکینی سالمندان: مرور یکپارچه

مقدمه و هدف: با توجه به پیچیدگی مراقبت از سالمندان، یک رویکرد چند رشته ای پیش نیازی برای ایجاد تعادل بین مداخله درمانی و تسکینی است. علیرغم توسعه و اجرای مدل‌های آموزش تیمی در سالمندان و مراقبت‌های تسکینی، توجه کمی به ماهیت و فرآیند کار تیمی شده است. مرور یکپارچه بر موانع و چالش های کار تیمی در مراقبت تسکینی در سالمندان می باشد.مواد و روش‏ها: مطالعه مرور یکپارچه حاضر بر اساس روش شناسی وایت مور انجام گردید. بدین منظور پایگاه‌های اطلاعاتی pubmed، scopus، web of sciences،  science direct و موتور جستجوگر  google scholar با کلیدواژه های ’end of life ،’palliative medicine’ ،’terminal care’ ، ’hospice care’ palliative care’ interdisciplinary’ multidisciplinary team’ بین سالهای  2000 تا 2023 مورد جست‌وجو قرار گرفتند. تمام مقالات بر اساس سال، هدف پژوهش، معیارهای ارزیابی انتقادی و نتیجه گیری خلاصه و دسته بندی شدند.یافته ها: موانع کلیدی برای همکاری بین رشته‌ای در مراقبت تسکینی در سه حوزه؛ 1) آموزش: فرآیند آموزشی ناکافی، عدم توسعه آموزش مراقبت تسکینی. 2) اجرا: چالش ارجاع سالمندان نیازمند مراقبت تسکینی، کمبود نیروی متخصص، فرهنگ سازمانی نامناسب. 3) سیاست: مراقبت از هم گسیخته، کمبود بودجه و موانع نظارتی شناسایی شد.نتیجه ‏گیری: همکاری بین رشته ای در مراقبت تسکینی در سالمندان مستلزم فراهم آوردن فرصتهای آموزش بین حرفه ای برای اعضای تیم، ضمانت اجرایی، سیاست‌ گذاری در زمینه مراقبت‌ تسکینی و تامین بودجه و منابع مالی می باشد.

barriers and challenges of team working in palliative care of the elderly: an integrated review

background and aims: a multidisciplinary approach is needed to balance therapeutic and palliative intervention options, given the complexity of geriatric care. despite the development and implementation of team education models in geriatrics and palliative care, little attention has been paid to the nature and process of teamwork. an integrated review of interdisciplinary team collaboration, barriers and challenges in palliative care in the elderly.materials and methods: the present integrated review study was conducted based on the methodology of whitmore. pubmed, scopus, web of sciences, science direct and google scholar search end of life, palliative medicine, hospice care, terminal care, 'palliative care', 'interdisciplinary' team', 'quality of life', 'elderly' were searched without time limit. all the articles were summarized and categorized based on the year, research objective, critical evaluation criteria and conclusion. the results of the previous steps were analyzed by applying data reduction, data display, and data comparison.results: key barriers to interdisciplinary collaboration in palliative care were identified in the three domains; 1) education: inadequate training process, lack of development of palliative care training, practice. 2) implementation: challenge of referring elderly people in need of palliative care, lack of specialist staff and inappropriate organizational culture. and 3) policy: fragmented care, lack of funding and regulatory barriers.conclusion: interdisciplinary team collaboration in palliative care in the elderly requires providing inter professional training opportunities for team members, executive assurance, policy making in the domains of palliative care, and funding and financial resources.
