مقایسه تاثیر دهان‌شویه کلرهگزیدین و بابونه بر بروز ضایعات دهان در بیماران بستری در آی‌سی‌یو: یک کارآزمایی بالینی تصادفی

زمینه و هدف: بهداشت دهان جزء جدایی‌ناپذیر توجهات پرستاری در بخش مراقبت ویژه است.هدف این مطالعه مقایسه تاثیردهان‌شویه کلرهگزیدین و بابونه بر بروز ضایعات دهان در بیماران بستری در آی‌سی‌یو بیمارستان هاشمی‌نژاد مشهد می‌باشد. مواد و روش‌ها:این پژوهش یک کارآزمایی بالینی دوسوکور می‌باشد.در این پژوهش70 بیمار تازه بستری اینتویه در بخش آی‌سی‌یو بیمارستان هاشمی‌نژاد مشهد در سال1395 مورد بررسی قرار گرفتند.بیماران با روش قرعه‌کشی در دو گروه35 نفره تقسیم شدند.در گروه اول دهان‌شویه کلرهگزیدین و در گروه دوم از بابونه جهت دهان‌شویه استفاده شد.ارزیابی دهان به مدت4 روز انجام شد.جمع‌آوری اطلاعات با استفاده از اطلاعات دموگرافیک،ارزیابی نمره دهان با ابزارmps انجام گردید.اطلاعات با استفاده از نرم‌افزارspss16 مورد تحلیل قرار گرفت.یافته‌ها:نتایج نشان داد دهان‌شویه بابونه و کلرهگزیدین بر ضایعات دهان موثر است.در روز اول اختلاف معناداری به لحاظ ضایعات دهان در دو گروه وجود نداشت(p<0.05)ولی در روز دوم و سوم اختلاف دو گروه کلرهگزیدین و بابونه از نظر بروز ضایعات دهان معنادار بود(p<0.05)روز چهارم تفاوت معناداری در میزان اختلال در لثه و مخاط دهان در دو گروه وجود نداشت(p<0.05).میزان ضایعات دهان در روز چهارم در هر دو گروه به صورت چشم‌گیری کاهش یافت و ضایعات دهان با اختلال متوسط به اختلال خفیف تغییر یافتند. نتیجه گیری:دهان‌شویه بابونه و کلرهگزیدین در بهبود و کنترل ضایعات دهان و پلاک دندان موثر هستند.اگرچه کلرهگزیدین تاثیر سریع‌تری بر بهبود اختلال مخاط دهان و لثه دارد،اما بابونه در درازمدت تاثیری برابر با کلرهگزیدین دارد. بنابراین با توجه به مزایای دهان‌شویه گیاهی می‌توان مصرف آن را به عنوان جایگزین توصیه نمود.

comparison of the effect of chlorhexidine and chamomile mouthwash on the incidence of oral lesions in patients hospitalized in icu: a randomized clinical trial

background and aims: oral hygiene is an important part of nursing care in the intensive care unit. the aim of this study is to compare the effect of chlorhexidine and chamomile mouthwash on the incidence of oral lesions in patients admitted to the icu of hasheminejad hospital in mashhad.materials and methods: this research is a double-blind clinical trial. in this research,70 newly hospitalized patients with intoye were examined in the icu department of hasheminejad hospital in mashhad in 2015. the patients were divided into two groups of 35 people by lottery. in the first group, chlorhexidine mouthwash and in the second group,chamomile it was used for mouthwash. mouth evaluation was done for 4 days. data collection was done using demographic information, mouth score evaluation was done with mps tool. the data was analyzed using spss16 software.results: the results showed that chamomile and chlorhexidine mouthwash is effective on oral lesions. on the first day, there was no significant difference in terms of oral lesions in the two groups (p<0.05), but on the second and third days, there was a difference between the chlorhexidine and chamomile groups in terms of the occurrence of lesions. the mouth was significant (p<0.05). on the fourth day, there was no significant difference in the amount of disorders in the gums and oral mucosa in the two groups (p<0.05).the amount of oral lesions on the fourth day in both groups significantly decreased. and oral lesions were changed from moderate disorder to mild disorder.conclusion: chamomile mouthwash and chlorhexidine are effective in improving and controlling oral lesions and dental plaque. although chlorhexidine has a faster effect on improving oral mucosa and gum disorders, chamomile has the same effect as chlorhexidine in the long run. therefore, according to the benefits of herbal mouthwash, its use can be recommended as an alternative.
