زجر بی‌زبانی و تلاش برای ابراز نیاز در بیماران تحت تهویه مکانیکی هوشیار بستری در بخش مراقبت‌های ویژه: یک مطالعه کیفی

مقدمه: بیماران هوشیار تحت تهویه مکانیکی به‌خاطر داشتن لوله داخل تراشه و نیز به علل دیگر قادر به برقراری ارتباط نیستند و مشکلات زیادی را در این رابطه تجربه می‌کنند. مطالعه حاضر با هدف کشف تجربیات بیماران هوشیار تحت تهویه مکانیکی از ناتوانی در صحبت‌کردن و نیز کشف اشارات غیرکلامی استفاده‌شده توسط آن‌ها برای ابزار نیاز انجام شد. مواد و روش‌ها: مطالعه تحلیل محتوای کیفی حاضر روی 15 بیمار تحت تهویه مکانیکی هوشیار بستری در بخش مراقبت‌های ویژه در سال 1395 در ایران انجام شد. برای گردآوری داده‌ها از مصاحبه‌های نیمه‌ساختاریافته و مشاهده استفاده شد. نمونه‌گیری به روش هدفمند با انتخاب افراد دارای تجربیات غنی انجام شد. از روش تحلیل محتوای کیفی نوع مرسوم برای تجزیه‌وتحلیل داده‌ها استفاده شد. یافته‌ها: چهار مضمون اصلی «ناتوانی در حرف‌زدن ریشه مشکلات، بلوک عجزآور کانال‌های ارتباط، اشارات درک‌نشده و تلاش برای برقراری ارتباط» از توصیف‌های عمیق مشارکت‌کنندگان حاصل شد. نتیجه‌گیری: بیماران هوشیار بخش مراقبت‌های ویژه مشکلات ارتباطی بسیاری را در حین بودن تحت تهویه مکانیکی متحمل می‌شوند، به‌طوری‌که قادر به ابراز نیازهای خود نیستند و این موضوع می‌تواند ترس، ناامیدی و آشفتگی را برای بیمار به همراه داشته است. این بیماران از روش‌های مختلف اعم از ایماواشاره، ضربه‌زدن به تخت و لب‌خوانی برای ابراز نیاز‌های خود استفاده می‌کنند.

The difficulty of being voiceless and attempt for expressing needs among conscious mechanical ventilation patients in ICU: a qualitative study

Introduction: Conscious mechanically ventilated patients are not able to communicate due to the internal tubes and many other reasons which cause them to experience many difficulties. In this regard, the present study aimed to explore the experiences of conscious mechanically ventilated patients, such as speech impairment and discovery of nonverbal communication used by them for the expression of their needs. Materials and Methods: This qualitative content analysis was conducted on 15 conscious mechanically ventilated patients admitted to the intensive care unit in Iran during 2016. The required data were collected through semistructured interviews and observations and the subjects were selected using the purposive sampling method from patients with enough experience in this regard. Moreover, the collected data were analyzed using conventional qualitative content analysis methods. Results: Based on the results of data analysis, four main themes were found in the deep descriptions of the participants. These themes were the inability to speak as the root of problems, inconvenient blockage of communication channels, unsuccessful nonverbal communication, and attempt for communication. Conclusion: Conscious patients in the intensive care unit have many communication problems while undergoing mechanical ventilation. Therefore, they are not able to express their own needs and this leads to fear and disappointment in them. These patients use different methods, such as communicating via signs, tapping on the bed, lip reading, and writing to express their needs.
