   تاثیر دو پروتکل حرکت درمانی با و بدون ماساژ بر دامنه ی حرکتی اندام تحتانی کودکان مبتلا به فلج مغزی انقباضی

چکیده    زمینه و هدف: فلج مغزی گروهی از اختلالات حرکتی دائم غیر پیشرونده هستند که در مراحل اولیه تکامل ایجاد می شوند. پژوهش حاضر به منظور بررسی تاثیر دو پروتکل حرکت درمانی با و بدون ماساژ بر دامنه ی حرکتی اندام تحتانی کودکان مبتلا به فلج مغزی انقباضی انجام شد.روش: بیست و دو کودک مبتلا به فلج مغزی انقباضی با میانگین سنی 5.25±1.56، قد 6± 44 / 102 و وزن 67 / 7± 4/ 44 از بین کودکان فلج مغزی به صورت هدفمند و دردسترس انتخاب و به صورت تصادفی به 2 گروه 11 نفری حرکت درمانی و ترکیبی حرکت درمانی و ماساژ تقسیم شدند. گروه حرکت درمانی، به مدت 6 هفته 3جلسه در هفته و 40 دقیقه برنامه ی حرکت درمانی بدون ماساژ داشتند و گروه ترکیبی به طور همزمان تحت برنامه حرکت درمانی و ماساژ ) 20 دقیقه حرکت درمانیو بعد از آن 20 دقیقه ماساژ ( قرار گرفتند. در این مطالعه دامنه ی حرکتی مفاصل قبل و بعد از مداخله با استفاده از گونیامتر یا گامی ارزیابی شد. برای تجزیه و تحلیل داده ها از نرم افزار spss نسخه 18 و آزمون های تی وابسته و تحلیل واریانس مرکبدر سطح معنا داری p <0/05 استفاده شد. یافته ها: براساس نتایج، پروتکل حرکت درمانی به تنهایی فقط باعث افزایش معنا دار دامنه ی حرکتی خم شدن مچ پا شده است p=0.001 اما روش ترکیبی باعث افزایش معنادار خم شدن مچ پا p=0.003 باز شدن مچ پا p=0.001 خم شدن زانو p=0.008 و باز شدن زانو p=0.002 شده است.نتیجه گیری: با توجه به نتایج این پژوهش، به درمانگر ها پیشنهاد می شود، جهت نتیجه درمانی بهتر از ماساژ همراه با تمرین درمانی به منظور افزایش دامنه ی حرکتی مفاصل استفاده کنند.

Abstract    abstractbackground & aim: cerebral palsy is a group of permanent non-progressive motor disorders caused bycongenital anomalies or brain damage in the early stages of development. the aim of this study was، theeffect of two protocols exercise therapy with and without massage on the lower extremity range of motionin children with spastic cerebral palsymaterials and methods: in this study، 22 children spasmodic cerebral palsy with an average age of5.25±1.56، height 102.44±6 and weight 44.7±4.67 were randomly selected from children with cerebralpalsy. they were randomly divided into two groups movement therapy and combined (movement therapyand massage). movement therapy group، for 6 weeks 3 sessions per week and 40 minutes) had no massageprogram، and the combined group simultaneously underwent a therapeutic and massage program (20 minutesof treatment and 20 minutes of massage after massage). in this study، the range of motion of the ankle beforeand after the intervention was evaluated using gynaemtram yagamy. for data analysis، spss software anddependent t test and factor analysis of variance analysis were used at a significant level of p <0/05.results: based on the results، the movement therapy protocol alone has only significantly increased therange of motion of ankle dorsi flexion (p=0.001)، but the combined method has significantly increased dorsiflexion (p=0.003). ankle plantar flexion (p=0.001)، knee flexion (p=0.008) and knee extension (p=0.002).conclusion: according to the results of this research، therapists are suggested to use massage withexercise therapy to increase the range of motion of the joints for better treatment results.
