مقایسه کیفیت عملکرد بالینی و مراقبت از بیماران در پرستاران شاغل در بخش‌های آنکولوژی و عمومی بیمارستان

مقدمه: مراقبت از بیماران سرطانی، به لحاظ نوع بیماری و نوع درمانی که دریافت می کنند، حائز اهمیت خاص است؛ با این حال، شرایط خاص بیماران و سختی کار در بخش انکولوژی، ممکن است بر عملکرد بالینی و کیفیت مراقبت پرستاران تاثیر گذار باشد. این مطالعه با هدف مقایسه عملکرد بالینی و کیفیت مراقبت در پرستاران شاغل در بخش‌های انکولوژی با پرستاران بخش‌های عمومی انجام شد.روش کار: در این مطالعه ی توصیفی- تحلیلی به روش نمونه گیری احتمالی دو مرحله ای طبقه ای و تصادفی ساده، تعداد  110 پرستار از بخش‌های انکولوژی و 110 پرستار از بخش‌های عمومی (بخش‌های داخلی و جراحی) شاغل در 3 بیمارستان‌ وابسته به دانشگاه علوم پزشکی مشهد در سال 1401 انتخاب شدند. عملکرد پرستاران در سه نوبت و در شیفت‌های مختلف (صبح، عصر و شب) با پرسشنامه عملکرد بالینی پرستاران (ncpq) و پرسشنامه کیفیت مراقبت‌ پرستاری (qualpac) ارزیابی شد. داده ها با نرم افزار spss نسخه 21 مورد تجزیه و تحلیل آماری قرار گرفتند. یافته ها: در پرستاران شاغل در بخش‌های انکولوژی و بخش‌های عمومی به ترتیب میانگین و انحراف معیار نمره عملکرد بالینی 10.59±74.57 و 8.62±84.49(رضایت بخش) و نمره عملکرد کیفیت مراقبت 39.79±243.10 و 30.68±283.71 (متوسط) بود. بین دو گروه از نظر نمره عملکرد (0.001>p) و کیفیت مراقبت (0.001>p) تفاوت معنی دار مشاهده شد. از نظر کیفیت مراقبت پرستاری در پرستاران بخش های انکولوژی و عمومی،  به ترتیب 0.9 درصد و صفر درصد کیفیت ضعیف، 75.5 درصد و 14.5 درصد متوسط و 23.6 درصد و 85.5 درصد کیفیت مراقبت پرستاری بسیار خوب داشتند (0.001>p). در زمینه ی کیفیت عملکرد، به ترتیب  16.4 درصد و 6.4 درصد متوسط و 83.6 درصد و 93.6 درصد کیفیت عملکرد بسیار خوب (0.019=p) داشتند. نتیجه گیری: عملکرد بالینی و کیفیت مراقبت پرستاری در پرستاران شاغل در بخش‌های انکولوژی به طور معنی داری کمتر از پرستاران بخش‌های عمومی است؛ که ممکن است ناشی از تنش‌های روانی و سختی‌های محیط کار باشد. لذا اقدامات مدیریتی و حمایتی به منظور ارتقای عملکرد بالینی و کیفیت مراقبت پرستاری در پرستاران شاغل در بخش‌های انکولوژیتوصیه می شود.

comparing the quality of clinical performance and patient care in nurses working in oncology and general hospital departments

introduction: caring for cancer patients is important in terms of the type of disease and the type of treatment they receive. however, the specific conditions of patients and the difficulty of working in the cancer department may affect the clinical performance and quality of care of nurses. this research was conducted with the aim of comparing clinical performance and quality of care in nurses working in oncology departments with nurses in general departments.materials and methods: in this descriptive-analytical study, the sampling method was two-stage, stratified and simple random. 110 nurses from cancer departments and 110 nurses from general departments (internal and surgical departments) working in 3 hospitals affiliated to mashhad university of medical sciences were selected in 1401. nurses’ performance was evaluated in three shifts in the morning, evening and night shifts with the nurses’ clinical performance questionnaire (ncpq) and nursing care quality questionnaire (qualpac). the data was statistically analyzed with spss version 21 software.results: in nurses working in oncology departments and general departments, the mean and standard deviation of the clinical performance score was 74.57±10.59 and 84.49±8.62, and the quality of care performance score was 243.10±39.79 and 283.71±30.68, respectively. a significant difference was observed between the two groups in terms of performance score (p<0.001) and quality of care (p<0.001). regarding the quality of nursing care, 0.9% and 0% had poor quality, 75.5% and 14.5% had average quality, and 23.6% and 85.5% had very good nursing care quality, respectively (p>0.001). in terms of the performance quality, 16.4% and 6.4% had medium, and 83.6% and 93.6% had very good performance quality (p=0.019).conclusion: the clinical performance and quality of nursing care in nurses working in cancer wards is significantly lower than nurses in general wards; which may be caused by psychological tensions and hardships in the work environment. therefore, management and support measures are recommended in order to improve the clinical performance and quality of nursing care in nurses working in cancer departments.
