چالش های کنترل عفونت در اورژانس پیش بیمارستانی: مطالعه مروری نقلی

مقدمه: پرسنل اورژانس پیش بیمارستانی در طی فعالیت حرفه ای روزانه خود در معرض انواع مخاطرات شغلی از جمله مخاطرات بیولوژیک و تماس با انواع عوامل بیماری زا می باشند. با توجه به اینکه ارتقای ایمنی شغلی، کاهش بیماری های شغلی و کنترل عفونت یک چالش مهم در نظام سلامت می باشد. از این رو مطالعه حاضر با هدف بررسی چالش های کنترل عفونت در اورژانس-   پیش بیمارستانی انجام شده است.روش کار: مقاله مروری نقلی حاضر با جستجو در پایگاه داده های sid-iranmedex-sccience direct-scopus-pubmed  با استفاده از کلید واژه های اورژانس پیش بیمارستانی، پرسنل اورژانس پیش بیمارستانی، کنترل عفونت در بازه زمانی با هر دو زبان فارسی و انگلیسی از 1390 الی 1402 انجام گردید. مقالات پس از انتخاب براساس معیارهای ورود به مطالعه با استفاده از چک لیست استروبی و کانسورت از نظر کیفیت ارزیابی شده و در نهایت از میان 45 مقاله انتخاب شده در جستجوی اولیه، 25 مقاله وارد مطالعه شدند.یافته ها: براساس بررسی 25 مقاله وارد شده به مطالعه، چالش های کنترل عفونت در میان پرسنل اورژانس پیش بیمارستانی عبارت بودند از: 1-وجود موقعیت های متعدد مواجهه با انواع میکروارگانیسم ها، 2- بیماری های عفونی نوپدید و بازپدید،3- عدم آگاهی و آموزش کافی پرسنل اورژانس پیش بیمارستانی در زمینه رعایت احتیاطات استاندارد، 4- کمبود امکانات و تجهیزات حفاظت فردی پرسنل اورژانس پیش بیمارستانی. نتیجه گیری: چالش های متعددی در زمینه کنترل عفونت در اورژانس پیش بیمارستانی وجود دارد. این چالش ها می توانند این بخش با ارزش از نظام سلامت را متحمل آسیب های جانی و مالی متعدد نمایند. کلید اصلی کنترل عفونت در اورژانس پیش- بیمارستانی، احتیاطات استاندارد می باشد. با در نظر گرفتن این موضوع و انجام اقدامات لازم پیرامون آن، می توان به طور چشمگیری میزان کنترل عفونت در اورژانس پیش بیمارستانی را علی رغم چالش های متعدد موجود، بهبود بخشید.    

challenges of infection control in prehospital emergency: a descriptive review

introduction: pre-hospital emergency personnel are exposed to various occupational hazards during their daily professional activities, including biological hazards and contact with various pathogenic agents. considering that improving occupational safety, reducing occupational diseases and controlling infection is an important challenge in the health system. therefore, the present study was conducted with the aim of investigating the challenges of infection control in the pre-hospital emergency room.methodology: the present descriptive review article by searching sid-iranmedex-science direct-scopus-pubmed databases using the keywords of pre-hospital emergency, pre-hospital emergency personnel, infection control in the period of time with both farsi and english languages. it was done from 1390 to 1402. after the selection, the articles were evaluated based on the inclusion criteria using the strobe and consort checklist, and finally, among the 45 articles selected in the initial search, 25 articles were included in the study.findings: based on the review of 25 articles included in the study, the challenges of infection control among pre-hospital emergency personnel were: 1- the presence of many situations of exposure to various types of microorganisms, 2- emerging and re-emerging infectious diseases, 3- lack of sufficient awareness and training of pre-hospital emergency personnel in the field of compliance with standard precautions, 4- lack of facilities and personal protective equipment for pre-hospital emergency personnel.conclusion: there are many challenges in the field of infection control in pre-hospital emergency. these challenges can cause this valuable part of the health system to suffer numerous human and financial damages. the main key to infection control in the pre-hospital emergency is standard precautions. by considering this issue and taking the necessary measures around it, it is possible to significantly improve the level of infection control in the pre-hospital emergency, despite the many existing challenges.
