بررسی کیفیت اجرای فرایند پرستاری توسط پرستاران شاغل در بیمارستان شهید بهشتی کاشان در سال1399 : یک مطالعه توصیفی مقطعی

مقدمه: فرایند پرستاری چارچوبی برای ارائه مراقبت با کیفیت موثر، هماهنگ و سازمان یافته برای بیماران می‌باشد. اجرای فرایند پرستاری منجر به ارتقا مراقبت پرستاری و هماهنگی در جهت ارائه‌ی خدمات پرستاری می‌شود. این مطالعه با هدف بررسی کیفیت اجرای فرایند پرستاری توسط پرستاران شاغل در بیمارستان شهید بهشتی کاشان انجام شد.روش : در این مطالعه توصیفی مقطعی، تعداد 270 پرستار شاغل در بیمارستان شهید بهشتی کاشان با توجه به معیارهای ورود، طی بازه زمانی تقریبا 6 ماه (از آذر 1399 تا اردیبهشت ماه 1400 ) به روش تصادفی ساده  وارد مطالعه شدند. جهت جمع‌آوری داده‌ها از ابزار محقق ساخته استفاده شد. پس از مرور گسترده متون و کتب مرتبط استخر گویه شامل 30 گویه تهیه شد و وارد مرحله روایی و پایایی گردید. در مرحله روایی، روایی صوری و محتوا با محاسبه cvrوcvi انجام شد که 7 گویه حذف یا ادغام شد و  پایایی ابزار با استفاده از ضریب الفای کرونباخ 0.94 محاسبه شد. سپس ابزار 23 گویه‌ای در 5 حوزه کلی شناخت، تشخیص، برنامه ریزی، اجرا و ارزشیابی دسته‌بندی شد. داده‌ها با کمک نرم‌افزار spss نسخه 24، در سطح توصیفی شامل میانگین، انحراف استاندارد و فراوانی انجام شد. تجزیه و تحلیل دو متغیره در جهت کشف ارتباط بین متغیر وابسته و متغیرهای دموگرافیک استفاده شد.یافته ها: نتایج نشان داده میانگین سنی پرستاران 33 سال بود که 68.9% متاهل، 62.2% زن و 90% دارای مدرک دانشگاهی لیسانس،بودند. 65.2 % از پرستاران فرایند پرستاری را در سطح متوسط اجرا کردند. به تفکیک مراحل مختلف فرایند پرستاری 61.9 % پرستاران، مرحله شناخت فرایند پرستاری را در سطح ضعیف اجرا کردند و کیفیت اجرا در مراحل تشخیص پرستاری، برنامه ریزی، اجرا و ارزشیابی در سطح متوسط و به ترتیب 45.9 ، 70.7، 64.4  و 62.6 درصد محاسبه شد. نتیجه گیری: یافته ها  نشان دادند که علی رغم این که میزان اجرای خوب فرایند پرستاری بسیار پایین بوده ولی بیش از 50 درصد پرستاران مراحل مختلف فرایند پرستاری را در حد متوسط اجرا می‌کردند. به نظر می رسد، جهت ارتقای کیفیت اجرای فرایند پرستاری باید دانش و عملکرد پرستاران در مرحله شناخت تقویت گردد تا بتوان از این سلاح علمی در ارائه مراقبت پرستاری با کیفیت و ارتقای پیامدهای بیمار بهره جست.

quality of nursing process implementation by nurses working in shahid beheshti hospital, kashan in 2020: a cross-sectional descriptive study

introduction: the nursing process is a framework for providing effective, coordinated and organized quality care for patients. the implementation of the nursing process leads to the promotion of nursing care and coordination in the direction of providing nursing services. this study was conducted with the aim of evaluating the quality of nursing process implementation in nurses working in shahid beheshti hospital, kashan.methods: in this cross-sectional descriptive study, 270 nurses working in shahid beheshti hospital of kashan were included in the study by a simple random method during a period of almost 6 months (from december 2020 to may 2021) according to the inclusion criteria. a researcher-made tool was used to collect data. after an extensive review of related texts and books, an item pool including 30 items was prepared and entered the validity and reliability stage. in the validity stage, face and content validity was done by calculating cvr and cvi, 7 items were deleted or integrated, and the reliability of the tool was calculated using cronbach’s alpha coefficient of 0.94. then, the 23-item tool was categorized into 5 general areas of cognition, diagnosis, planning, implementation and evaluation. the data were analyzed using spss version 24 software, at the descriptive level including mean, standard deviation and frequency. bivariate analysis was used to discover the relationship between dependent variable and demographic variables.results: the results showed that the average age of the nurses was 33 years, 68.9% were married, 62.2% were female, and 90% had a bachelor’s degree. 65.2% of the nurses performed the nursing process at an average level. according to the different stages of the nursing process, 61.9% of nurses performed the recognition stage of the nursing process at a poor level, and the quality of implementation in the stages of nursing diagnosis, planning, implementation and evaluation was at an average level, respectively 45.9%, 70.7%. %, 64.4% and 62.6% were calculatedconclusion: the findings showed that despite the fact that the level of good implementation of the nursing process was very low, more than 50% of the nurses implemented the different stages of the nursing process at an average level. it seems that in order to improve the quality of the nursing process, the knowledge and performance of nurses should be strengthened in the cognitive stage so that this scientific weapon can be used in providing quality nursing care and improving patient outcomes.
