مقایسه تاثیر کرایوتراپی و طب فشاری بر شدت درد ورود سوزن به عروق شریانی – وریدی بیماران همودیالیزی

مقدمه: بیماران مبتلا به نارسائی کلیه برای انجام شدن همودیالیز به دفعات با درد ورود سوزن به عروق ناحیه فیستول شریانی وریدی مواجه می شوند؛ یکی از اقدامات مهم پرستاری تسکین درد بیماران به روشهای غیر داروئی است لذا این تحقیق با هدف مقایسه تاثیر روش کرایوتراپی و طب فشاری بر شدت درد ورود سوزن به عروق شریانی وریدی بیماران تحت همودیالیز انجام شد.روش کار: این پژوهش یک مطالعه شبه تجربی تک گروهی است که به روش سرشماری بر 42 بیمار زن تحت درمان با همودیالیز در بیمارستان های منتخب شهر کرمان در سال 1396 انجام گرفت. جمع آوری داده ها با استفاده از پرسشنامه جمعیت شناختی و مقیاس عددی درد انجام شد. اندازه گیری درد طی سه جلسه متوالی همودیالیز، روز اول بدون مداخله، روز دوم و سوم بعد از مداخله کرایوتراپی و مداخله طب فشاری انجام شد، در هر مرحله پس از ثابت شدن سوزن وارد شده، میزان درد با استفاده از مقیاس عددی درد اندازه گیری شد. هر یک از مداخله کرایوتراپی و طب فشاری قبل از ورود سوزن به عروق شریانی- وریدی به مدت 10 دقیقه در نقطه هوگو دست مخالف فیستول اجرا شد.یافته ها: اکثر افراد در وضعیت بدون مداخله (52.38%) درد متوسط، بعد از مداخله کرایوتراپی (52.38%) درد خفیف و بعد از مداخله طب فشاری (54.77%) درد متوسط را احساس کردند. میانگین نمره درد در هر دو روش طب فشاری (2.43 ±4.19) و کرایوتراپی (2.17 ± 3.16) در مقایسه با روش بدون مداخله (1.82±6.52) کاهش داشت این تفاوت درروش کرایو تراپی با میانگین رتبه ای 1.38 در کاهش درد تاثیر بیشتری رانشان داد (0.001 = p).نتیجه گیری: به کارگیری روش کرایو تراپی بر نقطه هوگو دست مخالف فیستول بیش از روش ماساژ بر کاهش درد ورود سوزن موثر بوده است. توصیه می شود پرستاران به منظور تامین راحتی و آرامش بیماران با استفاده از روش غیر داروئی کرایو تراپی درد بیماران همودیالیزی را کاهش دهند.

Comparing the Effect of Cryotherapy and Acupressure on Pain Intensity of Arteriovenous Fistula Cannulation in Patients Hemodialysis

Introduction: Patients with kidney failure undergoing hemodialysis are frequently exposed to the hemodialysis needles are inserted into the arteriovenous fistulas; one of the important interventions in nursing care is use of nonpharmacological interventions for relief pain. Therefore, this study was conducted to compare the effect of cryotherapy and acupressure on the pain intensity of needle insertion into arterialvenous fistula in hemodialysis patients.Methods: This research is a singlegroup, quasiexperimental study by census sampling which was performed on 42 hemodialysis women patients at selected hospitals in Kerman city in 2017.Data collection conducted by demographic questionnaire and pain numerical pain scale. A measurement of pain was done during three consecutive hemodialysis sessions, the first day no intervention, cryotherapy and acupressure intervention was done on the second and third day. In every stage, after being fixed the needle into the arteriovenous fistula vessels, pain was measured using numerical scale. Each cryotherapy and acupressure intervention was performed for 10 minutes on the Hugo point of hand not having AV fistula (contralateral arm).Results: Most of patients experienced moderate pain without intervention (52.38%), mild pain with cryotherapy intervention (52.38%), and moderate pain after acupressure intervention (54.77%). The mean score of pain was decreased in both acupressure methods (4.19 plusmn; 2.43) and cryotherapy (3.16 plusmn; 2.17) compared to the no intervention technique (6.52 plusmn; 1.82). This difference was significant so that the cryotherapy with mean ordinal score of (1.38) was more effective in reducing pain (P = 0.001).Conclusions: Applying cryotherapy on Hugo point of contralateral hand AV fistula more than the acupressure has reduced pain of needle insertion. It is recommended to take into account comfort and relaxation of the patient, nurses using cryotherapy method to reduce pain for hemodialysis patients.
