مقایسه پارامترهای سونوگرافی داپلر در بیماران کلیه پیوندی با شرایط پایدار در زمان قبل و بعد از حداکثر مقدار سرمی سیکلوسپورین آ

مقدمه : سونوگرافی داپلر بهترین وسیله جهت ارزیابی وضعیت عروقی کلیه پیوندی می باشد. ازسوی دیگر سیکلوسپورین آ که به صورت گسترده جهت ممانعت از رد پیوند استفاده می شود، دارای اثرات انقباض عروقی می باشد که پارامتر های داپلر را تحت تاثیر قرار می دهد. تا به امروز، اتفاق نظر اندکی در مورد این اثر وجود دارد. هدف این مقاله بررسی تاثیر احتمالی سیکلوسپورین آ بر روی پارامترهای سونوگرافی داپلر می باشد. مواد و روش ها : مطالعه به صورت مقطعی بر روی 31 بیمار با شرایط پایدار، در فاصله زمانی 6 تا 18 ماه بعد از پیوند کلیه انجام گرفت. پارامتر های داپلر شامل شاخص مقاومت(resistive index)، شاخص پالستایل (pulsatility index) ، حداکثر سرعت سیستولیک (peak systolic velocity)، سرعت انتهای دیاستولیک (end diastolic velocity) ، در شریان اینترلوبار قبل و دو ساعت بعد(پیک سرمی) از مصرف خوراکی سیکلوسپورینآ بدست آمد. داده ها توسط نرم افزار آماری spss آنالیز شدند تا تاثیر احتمالی سیکلوسپورینآ بر روی پارامترهای داپلر بررسی شود. نتایج : مقایسه پارامترهای سونوگرافی داپلر قبل و دو ساعت بعد از مصرف سیکلوسپورینآ در بیماران کلیه پیوندی با متوسط کراتینین سرمی1.3 نشان داد که مقادیر داده ها بدون تغییرباقی می مانند. نتیجه گیری : نتایج حاصل از مطالعه ما نشان دادکه مصرف سیکلوسپورین آ، باعث افزایش حاد مقادیر پارامترهای سونوگرافی نمی شود.

Comparison of Doppler ultrasonographic indices before and after cyclosporine A peak serum level in stable kidney transplanted recipients Abstract: Background: Doppler ultrasonography is the best tool for assessment of transplanted kidneys rsquo; vascular status. On the other hand cyclosporine A widely consume to prevent graft rejection, has some vasoconstriction effects influence the Doppler parameters. To date, there has been little agreement on this effect. This paper will focus on the evaluation of whether cyclosporine A was affect the Doppler parameters. Materials and methods: A crosssectional study was performed on 31 stable patients, between 618 months after transplantation. All patients were received immunocompromised treatment including cyclosporine A. Interarenal Doppler indices, resistive index (RI) and pulsatility index and end diastolic velocity (EDV) and peak systolic velocity (PSV) were measured at interlobar artery level before and 2 hour after (peak serum level) the oral intake of cyclosporine A. Data analysis were made by SPSS software to estimate the influence of cyclosporine A on the Doppler parameters. Results: compared with Doppler parameters before and 2hour after cyclosporine A, among patients with mean serum creatinin levels about 1.3, values remained unchanged. Conclusion: In this study, consumption of cyclosporine A dose not lead to acute rise in ultrasonographic Doppler indices.
