بررسی تاثیر مکمل گلوتامین در مقایسه با ایزو وی بر علائم بالینی و کیفیت زندگی بیماران مبتلا به سندرم روده تحریک پذیر

چکیده
|
سابقه و هدف: سندرم روده تحریک پذیر یک اختلال عملکردی شایع روده میباشد که با وجود بار سنگین اقتصادی و روانی و کاهش شدید کیفیت زندگی در بیماران مبتلا به این سندرم، متاسفانه هنوز درمان قطعی برای این اختلال وجود ندارد. با توجه به این که گلوتامین قادر به کاهش نفوذ پذیری بیش از حد روده و همچنین تغییر در فلور میکروبی روده میباشد و احتمالاً موجب بهبود عملکرد سد رودهای و جمعیت میکروبی دوستدار روده میگردد، این مطالعه با هدف بررسی اثر مکمل گلوتامین در مقایسه با پروتئین ایزو وی بر علائم بالینی و کیفیت زندگی بیماران مبتلا به سندرم روده تحریکپذیر صورت گرفت.مواد و روشها: این مطالعه به صورت کارآزمایی بالینی تصادفی دوسو کور با 42 بیمار مبتلا به سندرم روده تحریک پذیر 18 تا 70 سال از هر دو جنس زن و مرد به مدت 6 هفته انجام شد. بیماران به صورت تصادفی به دو گروه تقسیم شدند. یک گروه، مکمل گلوتامین و گروه دیگر مکمل ایزو وی را به صورت 5 گرم سه بار در روز مصرف نمودند. امتیاز شدت علائم بالینی و کیفیت زندگی داوطلبان در ابتدا و انتهای مطالعه اندازهگیری شد.یافتهها: امتیاز کل شدت علائم بالینی (015/0=p)، تکرر دردهای شکمی (028/0=p) و نارضایتی از عادات روده ای (001/0 = p) در گروه گلوتامین کاهش معنیداری نسبت به گروه مصرف کننده ایزو وی داشتند. به طوری که در گروه گلوتامین شدت علائم بالینی، تکرر دردهای شکمی و نارضایتی از عادات رودهای به ترتیب به میزان 178، 34 و 37 امتیاز نسبت به ابتدای مداخله کاهش یافتند.نتیجه گیری: مکمل گلوتامین میتواند به عنوان یک درمان کمکی همراه با رژیم غذایی کم fodmaps (الیگوساکاریدها، دی ساکاریدها، مونوساکاریدها و پلی الهای تخمیرپذیر) برای تسکین بیشتر علائم پیشنهاد گردد.

Effects of Glutamine Supplementation against Iso Whey Protein on Clinical Symptoms and Quality of Life of the Patients with Irritable Bowel Syndrome

Abstract
|
Background and Objectives: Irritable bowel syndrome is a functional gastrointestinal disorder. Despite heavy economic and psychological burdens and severe decreases in quality of life of the patients, no certain cure is available for this common disease. Glutamine can decrease intestinal permeability and possibly improve intestinal barrier function and intestinalfriendly microbiota. Therefore, the aim of this study was to assess effects of glutamine supplementation against iso whey protein on clinical symptoms and quality of life of the patients with irritable bowel syndrome. Materials Methods: This study was a doubleblind, randomized clinical trial; in which, 42 patients aged 18 -70 years of the two sexes were participated for six weeks. Patients were randomized to receive glutamine (5 g, three times a day) or iso whey for six weeks. Scores of the irritable bowel syndrome symptom severity and quality of life scores were calculated at the beginning and end of the study.Results: Total severity score of the irritable bowel syndrome (p = 0.015), dissatisfaction with bowel habit (p = 0.001) and abdominal pain frequency (p = 0.028) significantly decreased in the glutamine group, compared to the iso whey group. Moreover, the total score of the irritable bowel syndrome symptom severity, abdominal pain frequency and dissatisfaction with bowel habits respectively decreased by 180, 32 and 38 points, compared to the glutamine group at the beginning of the intervention.Conclusion: Based on the findings, glutamine supplementation can be suggested as an adjunct therapy with a low FODMAP diet (oligosaccharides, disaccharides, monosaccharides and fermentable polyols) to further relieve symptoms of the patients with irritable bowel syndrome.
