تبیین چالش های ارائه خدمات به افراد اقدام کننده به خودکشی در مراجعه به اورژانس: یک مطالعه کیفی

هدف: مدیریت بیماران اقدام به خودکشی، به ویژه در محیط پراسترس اورژانس، با چالش‌های پیچیده‌ای همراه است. این چالش‌ها شامل مسائل اخلاقی، انگ‌های اجتماعی، ملاحظات قانونی و موانع ساختاری است که بر کیفیت مراقبت‌های ارائه شده تاثیر می‌گذارد. پژوهش حاضر با هدف شناسایی و بررسی دقیق این چالش‌ها در میان ارائه دهندگان خدمات بهداشتی در اورژانس بیمارستان‌های دانشگاه علوم پزشکی جهرم انجام شده است تا راهکارهایی برای بهبود مراقبت از این بیماران ارائه دهد.روش‌ها: این مطالعه کیفی از رویکرد تحلیل محتوای مرسوم برای بررسی تجربیات و دیدگاه‌های کارکنان بخش اورژانس، از جمله متخصصان اورژانس، پزشکان و پرستاران استفاده کرد. شرکت‌کنندگان با استفاده از نمونه‌گیری هدفمند انتخاب شدند و معیارهای ورود به مطالعه حداقل سه سال تجربه در مدیریت اقدام به خودکشی بود. اشباع داده ها حجم نمونه را تعیین کرد. داده ها از طریق مصاحبه های نیمه ساختاریافته و انفرادی با سوالات باز جمع آوری شد. مصاحبه ها با استفاده از نرم افزار maxqda ضبط، رونویسی و تجزیه و تحلیل شد.یافته‌ها: مضامین اصلی و فرعی مطالعه شامل؛ نقض رازداری، انگ های فرهنگی پیرامون بیماری روانی، نگرانی های قانونی مربوط به مراقبت از بیمار، و شکست های سیستمیک در محیط مراقبت های اورژانسی بود. ملاحظات اخلاقی، به ویژه حفظ استقلال و حفظ حریم خصوصی بیمار، مسائل مهمی بود که توسط شرکت کنندگان برجسته شد. علاوه بر این، چالش‌های عاطفی و شناختی که ارائه‌دهندگان مراقبت‌های بهداشتی با آن مواجه بودند، مشهود بود که نشان‌دهنده پیچیدگی مراقبت در این موقعیت‌های پرمخاطره بود.نتیجه‌گیری: یافته‌های این مطالعه چالش‌های چند وجهی را که ارائه‌دهندگان مراقبت‌های بهداشتی در مدیریت بیماران خودکشی در شرایط اضطراری با آن مواجه می‌شوند، برجسته می‌کند. نتایج بر نیاز به اصلاحات سیستمیک، از جمله بهبود زیرساخت های مراقبت روانپزشکی، آموزش بهبود یافته برای ارائه دهندگان مراقبت های بهداشتی و اتخاذ رویکردهای مراقبتی حساس فرهنگی تاکید می کند. پرداختن به این چالش ها از طریق استراتژی های جامع می تواند منجر به بهبود قابل توجهی در کیفیت مراقبت ارائه شده به بیماران خودکشی شود و در نهایت نتایج بیماران را در شرایط اورژانس بهبود بخشد.

explaining the challenges of providing services to suicidal people in the emergency room: a qualitative study

introduction: managing patients who have attempted suicide poses significant challenges for healthcare providers, especially in emergency departments. these challenges include ethical dilemmas, cultural stigma, legal concerns, and systemic barriers that can compromise the quality of care. this study aims to identify and explore these challenges faced by healthcare providers in the emergency departments of jahrom university of medical sciences to inform strategies for improving patient care.methods: this qualitative study utilized a conventional content analysis approach to explore the experiences and perspectives of emergency department staff, including emergency medicine specialists, physicians, and nurses. participants were purposively selected based on a minimum of three years’ experience in managing suicide attempts. data saturation was used to determine sample size. semi-structured interviews were conducted to collect data, which were then audio-recorded, transcribed, and analyzed using maxqda software. graneheim and lundman’s content analysis method, involving open coding, categorization, and theme abstraction, was employed to analyze the data.results: the study identified several key challenges in managing suicidal patients, which were categorized into main themes and sub-themes. these included breaches in confidentiality, cultural stigmas surrounding mental illness, legal concerns related to patient care, and systemic failures within the emergency care environment. ethical considerations, particularly maintaining patient autonomy and privacy, were also significant issues highlighted by the participants. additionally, the emotional and cognitive challenges faced by healthcare providers were evident, reflecting the complexity of care in these high-stakes situations. discussion: this findings of this study highlight the multifaceted challenges that healthcare providers encounter in managing suicidal patients in emergency settings. the results underscore the need for systemic reforms, including the enhancement of psychiatric care infrastructure, improved training for healthcare providers, and the adoption of culturally sensitive care approaches. addressing these challenges through comprehensive strategies could lead to significant improvements in the quality of care provided to suicidal patients, ultimately improving patient outcomes in emergency settings.
