تظاهرات جدید پوستی ناشی از کووید-19؛ گزارش موردی

مقدمه: تظاهرات پوستی در تشخیص بیماری‌های مختلف عفونی نقش بسیار مهمی دارند. معرفی بیمار: یک خانم 67 ساله با تب (39.2 درجه سانتی گراد)، خستگی، ضعف عضلانی و تنگی نفس به بخش حوادث و فوریت های پزشکی بیمارستان تهران مراجعه کرد. وی از سه روز قبل از بستری شدن، زخم دهانی در سطح پشتی زبان و زخم مخاط باکال، ماکول های اریتماتوز و همچنین پاپول در کف دست، پا و ساق پا داشته است.نتایج: مقادیر شمار نبض، تعداد تنفس، فشار خون و اشباع اکسیژن به ترتیب 122 ضربه در دقیقه، 18 بر دقیقه، 119.78 میلی متر جیوه و 91 درصد بود. کووید19 مثبت یا واکنش زنجیره ای پلیمراز در زمان واقعی با سوآب حلق و بینی او تایید شد. سی تی اسکن قفسه سینه پنومونی سگمنتال را در هر دو ریه با درگیری متوسط ​​نشان داد. تست های آزمایشگاهی سطوح قابل توجهی از دیدیمر (3.5 میکروگرم بر لیتر)، لاکتات دهیدروژناز (469 واحد بر لیتر)، پروتئین واکنشی c (121 میلی گرم بر لیتر)، تعداد لنفوسیت ها (87٪) و تروپونین (0.065 نانوگرم/ میلی لیتر) را نشان داد. زمان پروترومبین (17 ثانیه)، زمان ترومبوپلاستین نسبی (45 ثانیه)، و سطوح آلانین آمینوترانسفراز (69 واحد بر لیتر)، آلکالین فسفاتاز (426 واحد بر لیتر)، فریتین (475 نانوگرم/میلی لیتر) و کراتینین (1.85 میلی گرم در دسی لیتر) ارزیابی شد. لوراتادین و نیستاتین به ترتیب برای درمان خارش موضعی و ضایعات مخاطی به مدت 5 روز تجویز شدند. سرانجام، وی پس از بهبودی در ششمین روز بستری با موفقیت از بیمارستان تحت شرایط پایدار با میزان اشباع اکسیژن 96% ترخیص شد.نتیجه گیری: این مطالعه نشان داد که اریتمی چندشکلی می‌تواند تظاهرات پوستی جدید بوجود آمده در اثر عفونت کووید19 باشد.

New Skin Manifestation Caused by Covid-19; Case Report

Introduction: Skin manifestations play a very important role in the diagnosis of various infectious diseases.Case Report: A 67yearold woman with fever (39.2 ° C), fatigue, muscle weakness, and shortness of breath referred to the Medical Emergency Department of Tehran Hospital. From three days before his hospitalization, he had oral ulcers on the back of the tongue and buccal mucosal ulcers, erythematous macules, as well as papules on the palms of the hands, feet, and legs.Results: Pulse rate, respiration rate, blood pressure and oxygen saturation were 122 beats per minute, 18 beats per minute, 119.78 mm Hg and 91%, respectively. Covid19positive or realtime polymerase chain reaction was confirmed with his pharyngeal swab. Chest CT scan showed segmental pneumonia in both lungs with moderate involvement. Laboratory tests significant levels of didimer (3.5 mu;g/L), lactate dehydrogenase (469 mg/L), Creactive protein (121 mg/L), lymphocyte count (87%) and troponin (065.0 ng/ml). Prothrombin time (17 seconds), partial thromboplastin time (45 seconds), and levels of alanine aminotransferase (69 units per liter), alkaline phosphatase (426 units per liter), ferritin (475 ng / ml) and creatinine (1.85 ml) G per deciliter) was assessed. Loratadine and nystatin were administered for 5 days to treat local pruritus and mucosal lesions, respectively.
