واضح سازی مفهوم سهمیه بندی ضمنی مراقبت پرستاری: تحلیل مفهوم با رویکرد واکر و آوانت

زمینه و هدف: سهمیه بندی ضمنی مراقبت های پرستاری یک نگرانی فزاینده در سیستم ارئه مراقبت های بهداشتی می باشد. سهمیه بندی مراقبت می تواند پیامدهای نامطلوبی برای بیماران به همراه داشته باشد. با توجه به اهمیت موضوع در نظام سلامت، شفاف سازی ابعاد مختلف این مفهوم ضرورت دارد. لذا این مطالعه با هدف واضح سازی مفهوم سهمیه بندی ضمنی مراقبت پرستاری با استفاده از رویکرد واکر و آوانت انجام شده است.روش ها: برای آنالیز مفهوم سهمیه بندی ضمنی مراقبت از رویکرد واکر و آوانت استفاده شد. جستجو در پایگاه های بین المللی داده شامل web of sciences ،pubmed ،scopus با استفاده از کلید واژه های implicit rationing, nursing care, implicit rationing of nursing care, rationing of nursing care و پایگاه های فارسی sid, iran medex, magiran  بدون محدودیت زمانی تا اکتبر 2023 انجام شد. درنهایت با توجه به معیارهای ورود و خروج 13 مقاله مرتبط با مفهوم مدنظر مورد آنالیز قرار گرفتند.نتایج: بررسی متون نشان داد که مفهوم سهمیه بندی ضمنی مراقبت پرستاری دارای پیچیدگی و ابعاد مختلفی می‌باشد. پیشایندهای این مفهوم  مربوط به بیمار، پرستاران و سازمان  می باشند. تصمیم گیری، اولویت بندی، پیامد، تعهد انجام مراقبت های پرستاری و اختلال در روند انجام مداخلات پرستاری ویژگی های تعیین کننده مفهوم سهمیه بندی ضمنی مراقبت پرستاری می باشند.نتیجه گیری: مطالعه حاضر ابعاد مختلف مفهوم سهمیه بندی ضمنی مراقبت پرستاری را مشخص نمود. مدیران و پژوهشگران پرستاری می توانند  از نتایج این مطالعه در طراحی اقدامات و مداخلات موثر بر کاهش بروز آن، استفاده نمایند.

clarifying the concept of implicit rationing of nursing care: concept analysis using walker and avant’s approach

background & aim: implicit rationing of nursing care is a growing concern in health care systems and can cause adverse consequences for patients. considering the importance of the issue in the health system, it is necessary to clarify the different dimensions of this concept. therefore, this study was conducted to clarify the concept of implicit rationing of nursing care using walker and avant’s approach.methods: walker and avant’s approach was used to analyze the concept of implicit rationing of care. the search was conducted in the web of science, pubmed, and scopus, sid, iran medex, magiran databases using the keywords of implicit rationing, nursing care, implicit rationing of nursing care, and rationing of nursing care without a time limit until october 2023. finally, according to entry and exit criteria, 13 relevant articles were analyzed.results: the review of the literature showed that the concept of implicit rationing of nursing care is a complex concept with numerous dimensions. the antecedents of this concept are related to patients, nurses, and the organization. decision-making, prioritization, outcome assessment, obligation to perform nursing care, and disruptions in the process of performing nursing interventions were identified as the determining features of the concept of implicit rationing of nursing care.conclusion: the present study showed that implicit rationing of nursing care was a multifaceted concept. nursing managers and researchers can use the results of this study to design effective measures and interventions to reduce its incidence.
