بلوک سمپاتیک کمری در دردهای مقاوم به درمان کوزالژی در بیمار با وابستگی به داروهای مخدری

زمینه و هدف:بلوک سمپاتیک کمری در دردهای با منشاء سمپاتیک اندام تحتانی بکار میرود. کوزالژی یا سندرم درد پیچیده ناحیه ای نوع دو (crps type 2) جزو سندرم های درد وابسته به فعالیت سمپاتیک محسوب می شود. درمان درد کوزالژِی بخصوص در بیماران با وابستگی به داروهای مخدری بسیار مشکل است .در بررسی مطالعات گزارشی مبنی بر کاربرد مداخلات درد در بیماران با وابستگی به داروهای مخدری پیدا نشد لذا در این مطالعه تجربه کاربرد بلوک سمپاتیک کمری روی درد مزمن و مقاوم بدرمان کوزالژی (crps type 2) در بیمار با وابستگی شدید به داروهای مخدری گزارش شده است. گزارش موارد: بیمار اقای 36 ساله که دچار ترومای crush injury اندام تحتانی چپ که با تشخیص کوزالژِی تحت بلوک سمپاتیک کمری قرار گرفت و دردهای سوزشی مقاوم به درمان اندام تحتانی از نمره درد (nrs) 10 به 5 کاهش یافت و نیز دردهای با کیفیت برق گرفتگی (shooting) اندام مبتلا بکلی از بین رفت .و مصرف داروی مخدری بیمار 50 درصد کاهش یافت. نتیجه گیری: بلوک سمپاتیک کمری می تواند در دردهای مقاوم بدرمان کوزالژی در بیمار با وابستگی به داروهای مخدری استفاده شود هرچند مطالعات بیشتر برای تاثیر دراز مدت این بلوک مورد نیاز است.

Lumbar Sympathetic Block for resistant to treatment causalgia in patient with opioid dependence.

AbstractBackground Aim:Lumbar sympathetic block is a commonly used technique for sympathetically mediated pain in lower extrimity. Causalgia (CRPS type 2 ) is also accepted to be associated with sympathetic system activation. Treatment of this condition is difficult, especially when the patient has opioid dependency. there has not been any report regarding lowerextremity lumbar sympathetic block in patient with causalgia and drug dependency. Therefore, in this study, our experience of using lumbar sympathetic block on chronic and refractory pain of causalgia (CRPS type 2) in a patient with severe dependence on opioid drugs has been reported.Case Report: the patient is 36 years old male with crush injury in left lower extrimity who underwent lumbar sympathetic block for causalgia and score of burning pain is reduced from 10 to 5 (NRS) and pain with quality of electric shock (shooting) completely disappeared . the patientchr(chr('39')39chr('39'))s opioid was reduced by 50%.Conclusion: Lumbar sympathetic block could be considered in treatment of lower limb causalgia in patient with opioid dependency. More reports and controlled trials are needed for further understanding the role of long term efficacy of Lumbar sympathetic block.
