چالش های پارادایم نیوتنی برای مدیریت نظام ارایه خدمات سلامت: بیان یک دیدگاه

با وجود آشکار شدن کاستی های پارادایم نیوتنی، هنوز هم این پارادایم تکیه گاه فکری، علمی و عملی بشر در بسیاری از زمینه هاست. دیدگاه نیوتنی یک دیدگاه تقلیل گراست و درک کل سیستم وابسته به درک اجزای آن می باشد. درک مدیران و رهبران سازمان ها از تقسیم بندی سیستم ها به ساده، بغرنج (complicated) و پیچیده (complex) و تفکیک این نظام ها از یکدیگر، برای بهبود و افزایش کارایی و اثربخشی دارای اهمیت دو چندان است. سیستم پیچیده نظام ارایه خدمات، اغلب به عنوان سیستم های ساده یا بغرنج مدیریت شده و اصول حاکم بر این نظام ها مانند اجرای دستورالعمل های سفت و سخت و نامرتبط با سیستم های پیچیده و انعطاف ناپذیر بودن برنامه ها، به اشتباه به نظام ارایه خدمات تعمیم داده و مدیران از آنها برای کنترل و مدیریت نظام پیچیده بهداشت و درمان استفاده می نمایند. همین نکته باعث شده که از توجه و تاکید بر تعامل اجزا و روابط آنها با یکدیگر، که از اصول مرتبط با نظام های پیچیده است و می توانند منجر به پایداری و بهبود اثربخشی و کارایی شود، غافل گردند. سیستم های پیچیده به جای کنترل مرکزی نیازمند کنترل توزیع شده بوده و به تاریخچه گذ شته خود وابسته بوده و درلبه های نظم و بی نظمی دارای شکوفایی شده و لزوما عوامل آنها درکی از سیستم بزرگ تر ندارند و همچنین به بازخورد واکنش نشان خواهد داد.

despite the evident shortcomings of the newtonian paradigm, it remains the intellectual, scientific, and practical foundation for many disciplines. this paradigm adopts a reductionist approach, where understanding the entire system hinges on understanding its constituent parts. for managers and organizational leaders, differentiating between simple, complicated, and complex systems, and recognizing their distinct characteristics, is vital for enhancing both efficiency and effectiveness. the complex healthcare delivery system is often managed as a simple or complicated system. managers mistakenly apply rigid guidelines and inflexible programs—principles more suited to simpler systems—to the healthcare system. this results in certain essential aspects being overlooked, such as the interaction between various components and their interrelationships, which are key characteristics of complex systems. focusing on these interactions could lead to increased sustainability, efficiency, and effectiveness. complex systems require decentralized control rather than central command. they are shaped by their historical context, thrive on the edge of order and chaos, and their components may not always have a complete understanding of the larger system. moreover, complex systems are notably responsive to feedback.
