پیامدهای مورد انتظار ازبه کارگیری دانش آموختگان پزشکی خانواده در سیستم سلامت ایران از دیدگاه ذی نفعان: یک مطالعه ی کیفی

زمینه و اهداف: هدف از تربیت و به کارگیری پزشکان خانواده در نظام سلامت، بهینه سازی سطح بندی خدمات و نظام ارجاع می باشد. مطالعه ی حاضر با هدف تبیین پیامدهای مورد انتظار از دانش آموختگان رشته ی تخصصی پزشکی خانواده از دیدگاه اساتید، دستیاران و مدیران ارشد و میانی نظام سلامت و مقایسه ی آن با پیامدهای تعریف شده در کوریکولوم آموزشی این رشته، در دانشگاه علوم پزشکی تبریز در سال 1398 انجام شد. مواد و روش ها: مطالعه ی حاضر از نوع کیفی با رویکرد تحلیل محتوا بوده و روش نمونه گیری غیرتصادفی و مبتنی بر هدف بود. برای جمع آوری داده ها از مصاحبه ی نیمه ساختارمند با شرکت 31 نفر در قالب 3 گروه از اساتید، دستیاران، مدیران ارشد و میانی نظام سلامت (به روش بحث گروهی(fgd) شامل 25 نفر و مصاحبه ی انفرادی شامل 6 نفر استفاده شد و مصاحبه ها تا رسیدن به اشباع داده ها ادامه یافت. تحلیل داده ها نیز با روش تحلیل محتوایی انجام شد. یافته ها: یافته های این مطالعه پس از مفهوم بندی، استخراج و کدبندی در 56 درون مایه (theme) در قالب 3 مضمون اصلی جایگاه شغلی (پزشک مسئول، سیاست گذار، ارائه دهنده ی خدمت)، وظایف شغلی (آموزش، پژوهش، مدیریت، مشاوره و ویزیت) و پیامدهای اشتغال (کاهش هزینه ها، کاهش ارجاع غیرضروری، ارتقای سلامت جمعیت) دسته بندی گردید. نتیجه گیری: به کارگیری پزشکان خانواده ی متخصص در جایگاه هایی همچون پزشک مسئول مجتمع سلامت و ارائه ی خدمت در مراکز جامع سلامت می تواند پیامدهایی همچون واقعی شدن ارجاع ها، کاهش هزینه ها و جلوگیری از خدمات غیرضروری و کاهش بار کاری سطوح 2 و 3 ارائه ی خدمت را به دنبال داشته باشد.

Expected Outcomes from Family Medicine Specialized Graduates in Iranian Health System from the Viewpoints of Stakeholders: A Qualitative Study

Background and Objectives: The purpose of training and applying family physicians in the health system is to optimize rating the services and referral system. The purpose of this study was to explanation of the expected outcomes of family medicine graduates from the viewpoints of professors, assistants and senior managers of the health system and its comparison with the defined consequences in the curriculum of this major at Tabriz University of Medical Sciences. Material and Methods: This is a qualitative study with the content analysis approach. Sampling method is nonrandom and purposive. Three focus group discussions and six semistructured interviews conducted with 31 participants, including three groups of professors, assistants and senior managers of the health system by FGD method and also including 25 participants and 6 individuals were used for individual interview. Interviews continued until data saturation. Content analysis method used for data analysis. Results: After conceptualizing, extracting and coding themes into 56 themes, the findings were categorized into three general themes: Occupational status (such as: Conscientious physician, policy maker, supervisor or provider), job duties (Education, research, management, consulting and visit) and employment outcomes (such as: Cost reduction, unnecessary refer and population health promotion). Conclusion: The use of specialist family physicians in positions such as the physician in charge of the health complex and service provision in comprehensive health centers can have outcomes such as appropriating referrals, reducing costs and preventing overuse, and reducing the workload of levels 2 and 3.
