آمادگی بیمارستان‌ها جهت تخلیه و انتقال بیماران بخش مراقبت‌های ویژه در حوادث و بلایا

مقدمه: بلایای طبیعی و انسانی مانند جنگ، سیل و زلزله از دلایل اصلی تخلیه بیمارستان‌ها می‌باشند. در این بحران‌ها، می‌بایست مسئولین بهداشتی از پیش برنامه‌های بیمارستان برای تخلیه کامل یا جزئی را برنامه‌ریزی نموده و فرماندهی و ساختار مدیریتی مطلوب را طراحی نمایند. با وجود خطر بالای تخلیه بخش‌های مراقبت‌های ویژه در موقعیت بحران، متاسفانه ارائه دهندگان خدمت در این بخش‌ها آموزش کافی در خصوص تخلیه ایمن و موثر بخش ویژه را دریافت نمی‌کنند.هدف: هدف از این مطالعه تعیین آمادگی بیمارستان‌ها جهت تخلیه و انتقال بیماران بخش مراقبت‌های ویژه در حوادث و بلایا می‌باشد.مواد و روش‌ها: در این مطالعه مروری که در سال 1398 انجام شد، به بررسی پروتکل‌های مرتبط با تخلیه بیماران بخش مراقبت‌های ویژه در زمان بلایا پرداخته شد. پایگاه‌های اطلاعات pubmed، scopus و isi (web of science) مورد بررسی قرار گرفتند. برای این منظور از کلمات کلیدی disaster، evacuation و intensive care unit استفاده شد. سپس پایگاه‌های داده با کمک واژگان کلیدی بررسی و جستجوی منابع انجام شد.یافته‌ها: در این مطالعه چهارده مرحله برای قبل، حین و بعد از تخلیه یک بخش مراقبت‌های ویژه در زمان بلایا در نظر گرفته شده است. این مراحل با توجه به مقالات مورد بررسی به صورت یک پروتکل معرفی شده است.بحث و نتیجه‌گیری: هنگامی که بیماران در بیمارستان در معرض خطر قرار می‌گیرند، مدیریت بیمارستان می‌بایست برای نحوه تخلیه تجهیزات و همچنین حفظ سلامتی بیماران و کارکنان تصمیم‌گیری کند. تخلیه بیمارستان روی بیماران، کارکنان، اعضای خانواده، جامعه و حتی کشور و همچنین پیامدهای موثر بر بیماران اثر قابل توجهی خواهد داشت. تخلیه موفقیت آمیز بخش مراقبت‌های ویژه در هنگام بروز فاجعه به آمادگی، مشارکت، برقراری ارتباط موثر و رهبری توسط ارائه دهندگان مراقبت‌های ویژه نیاز دارد.

Review of the Protocol for Evacuation and Transfer of Patients in the Intensive Care Unit in Disasters

Introduction: Natural or humanmade disasters are the main reasons for hospital evacuation. Health officials must preplan hospital programs for complete or partial evacuation and establish an optimal management structure for the area of ​​operation. In the present study, we outlined some points for dealing with the possibility of hospital discharge. Despite the high risk of the evacuation of intensive care units in crises, service providers in these wards unfortunately do not receive adequate training on the safe and effective evacuation of intensive care units.Objective: The present study aimed to determine the extent of readiness of hospitals to evacuate and transfer patients hospitalized in the intensive care unit in accidents and disasters.Materials and Methods: In the present review study conducted in 2019, a protocol for evacuating patients in the intensive care unit during disasters has been studied. PubMed and Scopus databases were reviewed. For this purpose, the keywords Disaster, Evacuation, and Intensive Care Unit were used. Then, databases were searched with the help of keywords. Results: In the present study, fifteen steps have been considered for before, during, and after the evacuation of an intensive care unit during disasters. These steps are introduced as a protocol according to the papers reviewed.Discussion and Conclusion: While patients are at risk, the hospital manager often decides on how evacuate facilities for protecting patients and staff. Hospital evacuation had a dramatic effect on staff, family members, the community, and even the country, as well as the consequences for patients. Successful evacuation of the ICU in the event of a disaster requires readiness, participation, effective communication, and leadership by special care providers.
