شناسایی موانع گزارش‌دهی خطاهای پزشکی از دیدگاه تکنولوژیست‌های اتاق عمل مراکز آموزشی و درمانی وابسته به دانشگاه علوم پزشکی ایران

مقدمه: اتاق عمل با توجه به انجام پروسیجرهای تهاجمی مختلف می‌تواند بستری برای وقوع انواع خطاهای پزشکی باشد. ثبت موارد خطا و گزارش به موقع بر پیشگیری از آن و افزایش ایمنی بیمار، از اهمیت بالایی برخوردار است.هدف: هدف این مطالعه تعیین موانع گزارش‌دهی خطاهای پزشکی از دیدگاه تکنولوژیست‌های اتاق عمل مراکز آموزشی و درمانی وابسته به دانشگاه علوم پزشکی ایران در سال 98-1397 بوده است.مواد و روش‌ها: مطالعه حاضر در سال 1398 به روش توصیفی-تحلیلی بر روی 152 تکنولوژیست اتاق عمل که به روش نمونه‌گیری در دسترس انتخاب شده بودند، انجام شد. ابزار شامل دو قسمت اطلاعات دموگرافیک و موانع گزارش‌دهی خطاهای پزشکی بود. داده‌های جمع‌آوری شده در این پژوهش با استفاده از نرم‌افزار آماری spss نسخه 22 مورد تجزیه و تحلیل قرار گرفت.یافته‌ها: میانگین کل موانع گزارش‌دهی خطا 1/98 با انحراف معیار 0/36 (از 3 نمره) بوده است. میانگین نمرات موانع گزارش‌دهی خطا در بین 4 حیطه مورد بررسی نشان می‌دهد که بیشترین موانع گزارش‌دهی خطا مربوط به حیطه عامل قانونی (0/49 ±2/14) و کمترین موانع گزارش‌دهی خطا نیز مربوط به عامل مالی (0/51 ±1/85) بوده است. بیشترین موانع گزارش‌دهی مربوط به »در معرض اتهام بودن فرد « و »ترس از مواخذه و بازخواست « و »حجم بالای کار « بوده است. نتایج آزمون تی‌ تست و آنالیز واریانس نشان داد که تفاوت معناداری بین موانع گزارش‌دهی خطا با جنسیت (0/026‌=p) و سابقه کاری (0/031‌=p) وجود داشت؛ اما بین موانع گزارش‌دهی خطا با سایر متغیرها ارتباط معناداری مشاهده نشد (p>0/05).بحث و نتیجه‌گیری: در معرض اتهام بودن فرد و ترس از پیامدهای گزارش‌دهی، از دلایل شایع عدم گزارش‌دهی خطا بود. توصیه می‌شود تعامل مناسبی بین تکنولوژیست‌های اتاق عمل و مسئولین فراهم گردد تا تکنولوژیست‌های اتاق عمل صادقانه و بدون هیچ ترسی خطای خود و دلیل بروز آن را به سایر تیم درمان بیان کنند.

Identification of Medical Errors Reporting Barriers from the Viewpoints of Operating Room Technologists in Educational and Therapeutic Centers Affiliated to Iran University of Medical Sciences

Introduction: The operating room can be used to treat various types of medical errors due to numerous invasive procedures. The effects of recording error messages and timely reporting on preventing it and improving patient safety are of great importance.Objective: The present study aimed to determine the barriers of medical reporting errors from the viewpoints of operating room technologists of the educational and therapeutic centers affiliated to Iran University of Medical Sciences in 2019.Material and Methods: The present study was a descriptiveanalytical and temporal crosssectional study performed on 152 operating room technologists selected using convenience sampling in 2019. The tool included demographic information and medical reporting errors. Data were analyzed using SPSS 22.Results: The Mean and standard deviation of total error reporting barriers was 1.98 ±0.36 (out of 3). The mean score of error reporting barriers in the four areas under review shows that the highest and lowest error reporting barriers results from legal domain (2.14 ±0.49) and financial factor (1.85 ±0.51), respectively The highesterror reporting barriers were subject to ldquo;personal accusation rdquo;, ldquo;fear of accusations and being charged of rdquo; and ldquo; Workload rdquo;. The results of the ttest and analysis of variance showed that there was a significant difference between the error reporting barriers, gender (P=0.026) and work experience (P=0.031). However, there was no significant correlation between error reporting barriers and other variables (P> 0.05).Discussion and Conclusion: Exposing the person to accusations and fearing the consequences of reporting was one of the most important obstacles in delivering reporting errors. In this regard, the most important step in removing this obstacle is to create a space in which each room technologist expresses errors honestly and without fear and the reason for its occurrence to the other treatment team. Therefore, it is recommended that proper interaction be established between the operating room technologists and the authorities.
