چالش های پرونده الکترونیک سلامت در مراقبت مادران باردار از دیدگاه ماماهای مراقب سلامت

هدف: پرونده الکترونیک سلامت می تواند کیفیت مراقبت سلامت افراد جامعه بخصوص مادران را بهتر نماید. از طرفی به کارگیری آن در مراکز مراقبت اولیه با چالش هایی مواجه شده است. مطالعه حاضر برای شناسایی تجربیات ماماهای مراقب سلامت از سامانه پرونده الکترونیک سلامت دانشگاه علوم پزشکی مشهد موسوم به « سینا » در مراقبت از مادران باردار انجام گرفت.روش ها: این مطالعه کیفی به روش تحلیل تماتیک انجام گردید. شرکت کنندگان ماماهای مراقب سالمت در مراکز بهداشت دانشگاه علوم پزشکی مشهد بودند. برای گردآوری نظرات از رویکرد مصاحبه بدون ساختار استفاده گردید که بدین ترتیب 14 ماما باتجربه یک سال کار با سامانه « سینا » در مراکز بهداشت مورد مصاحبه قرار گرفتند. تحلیل دادها بر اساس راهنمای راون و کالرک انجام گردید. نتایج: چالش های کار با سامانه در سه تم اصلی مدیریتی، اطلاعاتی و فنی شناخته و دسته بندی شد. چالش های مدیریت متشکل از دو تم فرعی فرایند کار و ساختار، چالش های اطلاعاتی درحوزه کیفیت و حجم آن بود. دو تم فرعی چالش های شبکه و نرم افزاری در حوزه فنی دسته بندی شدند. نتیجه گیری: سیستم های مراقبتهای بهداشتی باید به طور منظم پرونده الکترونیک سلامت را ارزیابی کنند تا با نیازهای در حال تغییر سازگار باشند. در این راستا توجه به کاربران و برآوردن نیازهای آنان، طراحی مجدد فرایند کار، افزایش همکاری بین ارائه دهندگان مراقبت های بهداشتی، سیاست گذاران و توسعه دهندگان ضرورت دارد. همچنین اجرای عملکردی سیستم ارجاع، ایجاد قابلیت های سامانه های الکترونیک، قابلیت تعامل و انتقال اطالعات با سایر سامانه های سطح کشور برای سامانه سینا ضرورت دارد.

challenges of the electronic health record in the care of pregnant mothers: healthcare midwives’ viewpoint

introduction: electronic health records can advance the quality of health care for society, especially mothers. on the other hand, implementation in primary care centers has faced challenges. the study was conducted to identify the experiences of health care midwives from the electronic health record system of mashhad university of medical sciences known as “sina” caring for pregnant.methods: this qualitative study was conducted using the thematic analysis method. the participants were healthcare midwives in the primary health centers of mashhad. an unstruc­tured interview approach was used to gather opinions, thus 14 midwives with one year of experience working with the “sina” system in primary health centers were interviewed. the analysis was done as described by braun and clarke.results: the challenges of working with the system were identified and categorized three main themes of management, information and technical challenge. management challenges consisted of two sub-themes of work process and structure, data challenges in the field of quality and volume. two sub-themes of network issues and software usability were catego­rized in the field of technical.discussion: healthcare systems must regularly evaluate electronic health records to adapt to changing needs. in this regard, it is necessary to pay attention to users and meet their needs, redesign the work process, and increase cooperation between healthcare providers, policymakers, and developers. also, the functional implementation of the referral system, the creation of electronic system capabilities, and the ability to interact and transfer information with other country-level systems are necessary for the “sina” system.
