چکیده
|
هدف: مادران کودکان مبتلا به تالاسمیماژور، به عنوان مراقبین اصلی، در تطابق با مشکلات و وظایف متعدد ناشی از بیماری کودک، دچار چالشهای بسیاری میشوند و نیاز به حمایتهای رواندرمانی دارند، بنابراین پژوهش حاضر با هدف بررسی اثربخشی رواندرمانی وجودی به شیوه گروهی بر کاهش فشارمراقبتی مادران کودکان تالاسمیماژور انجام گرفت.روش ها: در این مطالعه شبه آزمایشی 16 مادر دارای کودک مبتلا به تالاسمیماژور مراجعهکننده به بیمارستان شهیدمحمدی و مرکز بیماریهای خاص ابوریحان هرمزگان در تابستان 1398 شرکت کردند. مادران به شیوه در دسترس انتخاب و در دو گروه 8 نفره مداخله و شاهد قرار گرفتند. گروه مداخله هشت جلسه رواندرمانی وجودی به شیوه گروهی را طی 8 هفته دریافت نمودند. دادهها با استفاده از مقیاس سنجش فشارمراقبتی گردآوری و با استفاده از نرمافزار spss آنالیز شدند.نتایج: میانگین نمره کل فشارمراقبتی مادران قبل از مداخله در گروه مداخله و کنترل به ترتیب 12.25 ±67.37 و 17.23 ±64.8 (74/.=p-value) و بعد از مداخله به ترتیب 4.7 ±40.8 و 24.94 ±65.75 (01/.>p-value) بود. همچنین نمره فشارمراقبتی مادران در 5 بعد زمانی، تکاملی، جسمانی، اجتماعی و احساسی در گروه مداخله بهطور معناداری کمتر از گروه کنترل بود. میانگین فشارمراقبتی در مادران کودکان مبتلا به تالاسمیماژور، پس از اجرای مداخله در گروه آزمایش کمتر از میانگین گروه کنترل بود که این تفاوت از لحاظ آماری معنادار است (001/.>p-value). نتیجهگیری: نتایج بیانکننده آن است که درمان وجودی به شیوه گروهی میتواند بر کاهش فشارمراقبتی در مادران کودکان تالاسمیماژور موثر باشد.

Abstract
|
Introduction: Mothers of children with thalassemia major, as primary caregivers, face many challenges and need psychotherapy in response to various problems caused by their chldrenchr('39')s illness. Therefore, the present study aims to investigate the effectiveness of cognitive existential group therapy in reducing the caregiving burden of mothers of children with thalassemia major.Method: This quasiexperimental study was performed on 16 mothers of children with thalassemia major who referred to the Abu Rayhan Special Medical Center and Shahid Mohammadi hospital in Bandar Abbas in the summer of 2019. Mothers were selected using available sampling method and divided equally into intervention and control groups. The intervention group received eight sessions of cognitive existential group therapy during eight weeks. Data were collected using the Caregiver Burden Inventory (CBI) and analyzed by SPSS software.Results: Before the intervention, the averages of caregiving burden scores were 67.37 ±12.25 and 64.87 ±17.23 in the intervention and the control groups, respectively. After the intervention these averages were 40.8 ±4.7 and 65.75 ±24.94. The score of caregiving burden in five dimensions including temporal, evolutionary, physical, social, and emotional dimensions in the intervention group was significantly lower than that of the control group. After the intervention, the mean of caregiving burden in mothers of children with thalassemia major in the intervention group was lower than that of the control group and the difference was statistically significant (PValue<0.001).Conclusion: These results suggest that cognitive existential group therapy can be effective in reducing the caregiving burden in mothers of children with thalassemia major.Keywords: Caregiving burden, Cognitive existential group therapy, Thalassemia major, Mothers.
