اثربخشی درمان مبتنی بر پذیرش و پایبندی بر کارکرد شغلی- اجتماعی و پذیرش نشانگان بیماری در افراد مبتلا به نوروپاتی دیابتی دردناک: مطالعه کارآزمایی بالینی یک سو کور

مقدمه: یکی از مشکلات شایع در افراد دیابتی، درد نوروپاتی است. در این افراد، درد باعث کاهش عملکرد شغلی و اجتماعی فردی می‌شود.هدف: هدف مطالعه حاضر بررسی اثربخشی درمان مبتنی بر پذیرش و پایبندی بر پذیرش نشانگان بیماری و سازگاری اجتماعی و شغلی در بیماران مبتلا به نوروپاتی دیابتی بود.روش: این مطالعه از نوع شبه آزمایشی بود، که بیماران به صورت تصادفی در گروه مداخله و کنترل تقسیم شدند. شرکت کنندگان کلیه مراجعین دارای تشخیص نوروپاتی دیابتی مراجعه کننده به بخش نورولوژی بیمارستان فارابی کرمانشاه در سال 1398 بودند. ابزارها شامل پرسشنامه پذیرش درد (cpaq) و سازگاری شغلی اجتماعی (wsas) بود. نتایج در سه مرحله پیش‌آزمون، پس‌آزمون و پیگیری مورد بررسی قرار گرفتند. داده‌ها به وسیله‌ی آزمون‌های توصیفی، آنالیز واریانس مکرر و کای اسکوئر با نرم‌افزار spss نسخه 22 مورد نجزیه و تحلیل قرار گرفتند.یافته‌ها: نتایج حاکی از آن بود که در مرحله‌ی پس‌آزمون و پیگیری، درمان مبتنی بر پذیرش و پایبندی نسبت به گروه کنترل قادر به افزایش پذیرش درد، بهبود و سازگاری اجتماعی شغلی گردید. در متغیر پذیرش نتایج آزمون درون گروهی نشان داد که اثر زمان (f=6/48 ،p=0/003) و اثر تعاملی زمان و گروه (f=12/49 ،p=0/001) معنادار است. همچنین در متغیر سازگاری شغلی و اجتماعی اثر زمان (f=3/21 ،p=0/046) و اثر تعاملی زمان و گروه (f=3/74 ،p=0/028) گزارش شد (0/001

The effectiveness of acceptance and commitment therapy on socio-occupational compatibility and acceptance of symptoms in patients with painful Diabetic Neuropathy: A single-blind randomized clinical trial

Introduction: Neuropathic pain is a common complication of diabetic patients. In these people, the pain reduces the work and social adjustment.Aim: The aim of this study was to evaluate the effectiveness of acceptance and commitment therapy on chronic pain acceptance and the work and social adjustment in patients with painful diabetic neuropathy.Method: This study was a quasiexperimental study in which patients were randomly divided into intervention and control groups. Participants were all patients with a diagnosis of diabetic neuropathy referred to the neurology department of Farabi Hospital in Kermanshah in 2019. Instruments included Chronic Pain Acceptance Questionnaire (CPAQ) and the Work and Social Adjustment Scale (WSAS). The results of three phases of pretest, posttest and followup were evaluated. Data were analyzed by descriptive statistics, the repeated measures ANOVA, and chisquare tests using SPSS version 22.Results: The results demonstrate that in the posttest and followup phases, acceptance and commitment therapy could improve pain acceptance and the work and social adjustment in the intervention group as compared to controls. In the acceptance variable, the test results within the group showed that the effect of time (P=0.003, F=6.48) and the interactive effect of time and group (P=0.001, F=12.49) were significant. Also in the variables of work and social adjustment, the effect of time (P=0.046, F=3.21) and the interactive effect of time and group (P=0.028, F=3.74) were reported (p>0.001).Conclusion: Acceptance and commitment therapy can be used as a psychological intervention along with drug therapy to improve the work and social adjustment and acceptance symptoms in patients with painful diabetic neuropathy (PDN).
