اولویت‌بندی بیماران کووید-19 جهت بستری در بخش مراقبت‌های ویژه در شرایط کمبود تخت بیمارستانی: یک رویکرد تصمیم‌گیری چندمعیاره ترکیبی

زمینه و هدف: در زمان شیوع یک بیماری همه‌گیر، منابع از جمله تخت بیمارستان به‌شدت محدود شده و همه بیماران قابل درمان نیستند، درنتیجه جیره‌بندی تخت‌های بیمارستانی اجتناب‌ناپذیر است. بنابراین، هدف از این مطالعه ارائه روشی برای ارزیابی و اولویت‌بندی بیماران مبتلابه کرونا جهت پذیرش در بخش مراقبت‌های ویژه بود. روش بررسی: این مطالعه ازنظر روش گردآوری داده‌ها توصیفیپیمایشی بوده و در مقطع زمانی تابستان 1400 انجام شد. پس از تشکیل پنل خبرگی با حضور 4 پزشک متخصص، با ترکیب روش‌های تجزیه‌وتحلیل تصمیم چندمعیاره، یک رویکرد 3 مرحله‌ای برای ارزیابی و اولویت‌بندی بیماران کووید19 اتخاذ شد. در مرحله اول، معیارها و زیرمعیارهای موثر برای اولویت‌بندی بیماران توسط پنل خبرگی و پژوهش‌های مرتبط شناسایی و سلسله‌مراتب معیارها رسم شد. در مرحله دوم، روش بهترینبدترین فازی برای تعیین وزن معیارها و زیرمعیارها به کار رفت. درنهایت، در مرحله سوم، راه‌حل مصالحه ترکیبی برای اولویت‌بندی 10 بیمار کووید19 نیازمند، انجام و بحرانی‌ترین بیمار انتخاب شد.یافته‌ها: با توجه به ارزیابی خبرگان، در میان 15 معیار بررسی‌شده، تعداد تنفس، pao2/fio2، اختلال هوشیاری، میانگین فشار شریانی و سن به ترتیب به‌عنوان 5 تا از مهم‌ترین معیارها در اولویت‌بندی بیماران کووید19 جهت بستری در بخش مراقبت‌های ویژه تعیین شدند. علاوه بر این، 10 بیمار کووید19 مراجعه‌کننده به بیمارستان شهدای پاکدشت با رویکرد پیشنهادی ارزیابی و اولویت‌بندی شدند.نتیجه‌گیری: روش پیشنهادی می‌تواند به‌عنوان یک ابزار مفید در ارزیابی بیماران کووید19 جهت پذیرش در بخش مراقبت‌های ویژه و پشتیبانی از تصمیمات حیاتی پزشکان استفاده شود.

Prioritization of COVID-19 Patients for Admission in the Intensive Care Unit in the Context of Hospital Bed Shortages: An Integrated Multi-criteria Decision Making Approach

Background: When an epidemic occurs, resources, including hospital beds, are severely limited and not all patients can be treated; so, hospital bed rationing is inevitable. Therefore, the aim of this study is to provide a method for evaluating and prioritizing patients with COVID19 for admission to the intensive care unit.Methods: This was a descriptivesurvey study in terms of data collection method conducted in the summer of 2021. Following the formation of an expert panel with 4 specialists, a threestage approach to evaluating and prioritizing COVID19 patients was adopted by combining multicriteria decision analysis methods. In the first stage, effective criteria and subcriteria for patients rsquo; prioritization were identified by a panel of experts and related studies, and the hierarchy of criteria was drawn. In the second stage, the Fuzzy bestworst method was used to determine the weight of criteria and subcriteria. Finally, in the third stage, a combined compromise solution method was performed to prioritize 10 patients in need of COVID19 and the most critical patient was selected. Results: According to the experts, among the 15 criteria studied, respiratory rate, PaO2/FiO2, impaired consciousness, mean arterial pressure and age were identified as 5 of the most important criteria in prioritizing COVID19 patients for intensive care unit admission. In addition, 10 COVID19 patients referred to Shohadaye Pakdasht Hospital were evaluated and prioritized with the proposed approach.Conclusion: The proposed method can be used as a useful tool in the evaluation of COVID19 patients for admission to the intensive care unit and to support the vital decisions of physicians.
