ارائه الگوی اجرای موثر سیاست‌های نظام سلامت کشور

زمینه و هدف: خط مشی ها پس از تدوین و تصویب، برای اجرا به مدیران ابلاغ می گردند. گاهی در مرحله اجرا دستخوش تغییراتی می شوند که آن ها را از اهداف شان دور می سازد. هدف این پژوهش ارائه الگویی است که منجر به ارتقای کیفی مرحله اجرای سیاست های تدوین شده در نظام سلامت کشور گردد. روش پژوهش: مطالعه کیفی حاضر در سال 1397 انجام گرفت. برای جمع آوری اطلاعات در مرحله اول از روش کتابخانه ای با ابزار فیش برداری و در مرحله دوم میدانی، از ابزار مصاحبه های اکتشافی استفاده شد. جامعه مورد مطالعه شامل مدیران عالی وزارت بهداشت و روسای دانشگاه های علوم پزشکی و بیمارستان های دولتی بود. در این پژوهش از نمونه گیری هدفمند و گلوله برفی برای انتخاب مصاحبه شوندگان استفاده شده است و جمعاً 33 مصاحبه انجام شد. برای تجزیه و تحلیل داده ها از تحلیل داده بنیاد و برای مدل سازی از نگرش سیستمی استفاده گردید. همچنین برای آزمون مدل از مصاحبه های روایی سنجی استفاده گردید. یافته ها: پس از ثبت داده های گردآوری شده و حذف مولفه های تکراری مصاحبه های اکتشافی و پیشینه تحقیق، 115 گزاره شناسایی شد. پس از رمزگذاری باز، 115 مفهوم تعریف گردید. در نهایت به وسیله رمزگذاری محوری و کدگذاری انتخابی، تعداد این ابعاد به 9 مولفه (فرهنگ عمومی، ساختاری، اطلاع رسانی، کیفیت خط مشی، سبک و شیوه اجرا، قوانین و مقررات، تخصیص منابع، نیروی انسانی توانمند و محیط) رسید. این مولفه ها هر کدام دارای ابعاد و شاخص هایی بودند که در مجموع تعداد 46 شاخص مرتبط با ارتقای ظرفیت اجرای سیاست های نظام سلامت شناسایی شد.نتیجه گیری: ارتقای کیفی اجرای سیاست های تدوین شده، در لایه های مختلف نظام سلامت کشور منجر به ایجاد فضای اعتماد متقابل بین نظام سلامت کشور و سطوح گوناگون جامعه هدف، افزایش انگیزه، تقویت روحیه همکاری، تشریک مساعی و کار گروهی در مجریان و فراهم آوردن زمینه های مطلوب برای بروز خلاقیت و نوآوری در اجرای سیاست ها، ارتقای شاخص های سلامت در جامعه و کسب اعتبار نظام سلامت کشور خواهد شد.

A Model for Effective Implementation of Health Policy in Iran

Background: Policies will be communicated to administrators after being drafted and approved. Sometimes, they undergo changes at the implementation stage that remove them from their goals. The aim of this study was to provide a model to improve the quality of policy implementation in the health system of Iran.Methods: This qualitative study was conducted in 2018. In order to collect the required information, a library method was used by not taking tool and in the second field stage, exploratory interviewing tool was applied. The study population consisted of the senior managers in the Ministry of Health, heads of the medical universities, and government hospitals. In this research, targeted and snowball sampling methods were used to select the interviewees. A total of 33 interviews were conducted. For analysis of the data, the database analysis was used and for modeling a system attitude was applied. Also, for the purpose of model testing, narrative interviews were used.Results: After recording the collected data and removing the repetitive components of the exploratory interviews and research background,115 statements were identified. After the open encryption, 115 concepts were introduced. Finally, by axial and selective encodings, 9 components were determined: general culture, structure, information, policy quality, implementation style, rules and regulations, resource allocation, empowerment, and environment. Each of these components had dimensions and characteristics, which made a total of 46 indicators associated with capacity building of the health system policies.Conclusion: Improved quality of the policies' implementation in various layers of the health system leads to a mutual trust between the health system of Iran and various levels of the target community, increases motivation, strengthens the spirit of cooperation, improves sharing efforts and teamwork among the executives, and provides an optimal context for emergence of creativity and innovation in implementation of policies, promotes health indicators in the community, and gains credibility for the health system of Iran.
