investigation of informal healthcare payments after health transformation plan: a case study in the southeast of iran

Background: informal payment in many developing countries is a serious obstacle towards to improving healthcare system and has negative effects on the function of healthcare system. in order to reduce the informal payments and direct payments from patient's pocket, the health transformation plan (htp) executed. this study was conducted to assess the informal payments in the hospital related to government. methods: this study was a survey based on descriptive analytic method in zahedan, the capital of sistan-baluchistan province, in 2017. the study population included all patients discharged from all wards of the two general hospitals affiliated with zahedan university of medical science (zaums) in a one-month period (from september 23 to october 22), in which 500 patients were selected by multistage cluster sampling. data collection tool was a standard questionnaire that was conducted by interview method. the data were analyzed using by spss v22 and descriptive statistics. results: in this study 98% of patients were male, 484 (96.8%) lived in the urban area, and about 2% had informal payment. the average amount of informal payments was 51.48 $. the highest number of informal payment was in the maternity ward (n:5, 50%). the doctors didn't have any informal payment, and all of the participants disagreed this type of payments. conclusion: the rate of informal payments in the hospitals affiliated with zaums in southeast of iran after execution of htp is minimal. poor economic status of the patients and well executed htp are the possible explanations for minimal informal payments. solutions including constant supervision on employees and realization of the medical services tariffs and strengthening insurance companies are effective steps towards elimination of informal payments.

بررسی پرداخت های غیررسمی و عوامل موثر بر آن در بیمارستان های دولتی بعد از اجرای طرح تحول نظام سلامت

زمینه و هدف: پرداخت های غیررسمی در بسیاری از کشورهای در حال توسعه یک مانع جدی و اساسی در راه بهبود نظام سلامت محسوب می شود و اثرات نامطلوبی بر عملکرد نظام سلامت دارد. در این راستا طرح تحول نظام سلامت، با هدف کاهش پرداخت از جیب و کاهش پرداخت های غیر رسمی در کشور اجرا شد. مطالعه حاضر با هدف تخمین میزان پرداخت های غیر رسمی در بیمارستان های دولتی انجام گردید.روش کار: این مطالعه که از نوع پژوهش های پیمایشی بود به روش توصیفی تحلیلی در سال 1396 در دانشگاه علوم پزشکی زاهدان انجام شد. جامعه پژوهش، تمام بیماران بستری دو بیمارستان عمومی بزرگ شهر زاهدان بوده که تعداد 500 بیمار به روش تصادفی ساده متناسب با حجم انتخاب شدند. ابزار گردآوری داده ها پرسشنامه استاندارد بود که به روش مصاحبه داده ها گردآوری شدند. داده ها با استفاده از نرم افزارSPSS V22 و آمار توصیفی و آزمون کای اسکور تحلیل شد. نتایج: در این مطالعه 98 درصد مصاحبه شوندگان مرد بودند و 484 نفر(8/96 %) از بیماران در منطقه شهری سکونت داشتند. حدود 2 درصد از بیماران پرداخت غیررسمی داشته اند و میانگین پرداخت غیررسمی به پرستاران سه میلیون ریال بود. بالاترین میزان پرداخت غیر رسمی در بخش زایشگاه با حدود 50% بود. پزشکان هیچ گونه دریافت غیر رسمی نداشتند. همچنین تمام بیماران مورد بررسی مخالف این نوع پرداخت ها بودند.نتیجه گیری: میزان پرداخت غیر رسمی در بیمارستان های دولتی جنوب کشور پس از طرح تحول نظام سلامت به میزان کم و محدودی می باشد. شرایط اقتصادی ضعیف بیماران و اجرای مناسب طرح تحول نظام سلامت را می توان از عوامل پایین بودن پرداخت های غیر رسمی دانست. ارائه راهکارها از جمله نظارت مستمر بر عملکرد کارکنان، واقعی تر شدن تعرفه ها در کنار تقویت نهاد های بیمه ای، گامی موثر جهت حذف پرداخت های غیر رسمی می باشد.کلمات کلیدی: طرح تحول نظام سلامت، پرداخت غیر رسمی، بیمارستان
