the relationship between the kind of delivery and 5 indexes of mother and baby's health before and after implementing health reform plan in selected hospitals of east azerbaijan (iran)

Background: to improve the individuals' health, reforms should be made in the health systems. as a result, assessing the impacts of reforms is a way to evaluate the effectiveness of the health system. the aim of this study was to study the relationship between the kind of delivery and the five indexes of mother and baby's health before and after implementing the health reform plan in selected hospitals of east azerbaijan, iran. methods: this descriptive-analytical study was conducted in 2018. the study was conducted among eight educational, medical, and private hospitals. before and after the program, 800 files were selected using simple random sampling method. the researcher made a check list with confirmed validity to extract the data. the data were analyzed using descriptive statistics and t-tests by spss-22. results: the average age of mothers in the two intervals was 27 years. most participants did not deliver before and their education was diploma or lower. the rate of natural delivery from 34.5% (before the plan) reached 44.2% (after the plan)(p<0.05). the rate of mothers and babies' mortality reduced from 0.3 and 0.8 to 0 and 0.5, respectively. mothers and babies' mortality and stillbirth had no significant difference based on the kind of delivery (p>0.05). conclusion: findings indicated significant increase of natural delivery after the reform plan in health system. moreover, the rate of mothers and babies' mortality decreased. these results can guide the policymakers for deciding about the course of plan and its review.

بررسی ارتباط بین نوع زایمان با میزان شاخص های پنج گانه سلامت مادر و نوزاد قبل و بعد از اجرای طرح تحول نظام سلامت در بیمارستانهای منتخب استان آذربایجان شرقی

مقدمه و هدف: نظام های سلامت به منظور تامین و ارتقای سلامت، اقدام به انجام اصلاحات می نمایند. سنجش اثرات اصلاحات یکی از روش های تعیین اثربخشی بوده و زمینه ساز بهبود وضعیت خواهد بود. لذا هدف از این مطالعه، بررسی ارتباط بین نوع زایمان با میزان شاخص های پنج گانه سلامت مادر و نوزاد در بازه زمانی قبل و بعد از اجرای طرح تحول نظام سلامت در بیمارستانهای منتخب استان آذربایجان شرقی می باشد.موارد و روش ها: پژوهش حاضر یک مطالعه توصیفی تحلیلی بوده که در سال 1396 انجام گرفت. محیط پژوهش شامل 8 بیمارستان آموزشی، درمانی و خصوصی بود. 800 پرونده در دو مقطع زمانی قبل و بعد از اجرای طرح به صورت تصادفی ساده انتخاب شدند. از چک لیست محقق ساخته که روایی آن توسط متخصصین امر تعیین شد، برای استخراج داده ها استفاده شد. داده های بدست آمده به وسیله SPSS22 و با بهره گیری از آمار توصیفی و آزمون های تی تست و کای دو تجزیه و تحلیل شد.یافته ها: میانگین سنی مادران در هر دو بازه زمانی 27 سال بود. اکثر مشارکت کنندگان فاقد زایمان قبلی بوده و دارای تحصیلات دیپلم و زیر دیپلم بودند. میزان زایمان طبیعی از 5/34 درصد (قبل از طرح تحول) به 2/44 درصد (بعد از طرح تحول) افزایش یافت (05/0>p). میزان مرگ مادران و نوزادان به ترتیب از میزان 3/0 و 8/0 درصد به صفر و 5/0 درصد کاهش یافت. مرگ و میر مادران و نوزدان و مرده زایی بر اساس نوع زایمان فاقد اختلاف معنی دار بود (05/0<p).نتیجه گیری: یافته های بدست آمده حاکی از افزایش معنی دار درصد زایمان طبیعی با اجرای طرح تحول در نظام سلامت ایران بوده است. همچنین میزان مرگ و میر مادران و نوزادان کاهش یافته است. این نتایج می تواند راهنمایی برای سیاستگذاران جهت تصمیم گیری در مورد ادامه اجرای برنامه و بازبینی آن باشد.کلمات کلیدی: طرح تحول نظام سلامت، شاخص های سلامت مادر و نوزاد
