طراحی محتوای اطلاعاتی برای ثبت دلایل لغو اعمال جراحی در سیستم اطلاعات بیمارستان

مقدمه: لغو یک عمل جراحی نه تنها هزینه زیادی بر سیستم ارائه دهنده مراقبت تحمیل می کند بلکه انرژی و وقت زیادی از پرسنل و بیمار را نیز به هدر می دهد. این مطالعه با هدف تعیین دلایل لغو اعمال جراحی و طراحی محتوای اطلاعاتی برای مستندسازی آنها در سیستم اطلاعات بیمارستان انجام شده است.روش: پژوهش توصیفی حاضر که یک مطالعه کاربردی می باشد در دو بخش اجرا شد. در بخش اول، کلیه پرونده های پزشکی بیماران بستری در بیمارستان آموزشی شفا که در یک بازه زمانی 6 ماهه، کد لغو عمل جراحی بر اساس icd10 به آنها اختصاص داده شده بود، بررسی شد. داده ها با استفاده از چک لیست پژوهشگرساخته گردآوری و با نرم افزار spss.v18 تجزیه و تحلیل شد. بخش دوم، مطالعه مروری مقالاتی است که دلایل لغو عمل را فهرست کرده بودند. به منظور بازیابی مطالعات، پایگاه های google scholar و pubmed جستجو شد. سپس نتایج حاصل از دو بخش، تطبیق داده شد و لیستی جامع از کلیه دلایل لغو عمل جراحی ارائه گردید.نتایج: از مجموع 7529 عمل جراحی انجام شده، در 1229 مورد (4/16 درصد)، عمل جراحی لغو شده بود. بیشترین دلیل لغو عمل جراحی، نامناسب بودن وضعیت بالینی بیمار قبل از عمل جراحی بود. دلایل استخراج شده از مطالعات در هشت گروه طبقه بندی و در مجموع، فهرستی شامل 54 دلیل برای لغو عمل جراحی تدوین شد.نتیجه گیری: مستند سازی دقیق دلایل لغو اعمال جراحی در هر مرکز درمانی به منظور شناسایی و در نهایت برنامه ریزی برای پیشگیری از آن ضروری می باشد. طراحی یک سیستم اطلاعاتی با محتوای متناسب می تواند این امر را محقق سازد.

Developing Information Content for the Documentation of Surgery Cancellation Reasons in Hospital Information System

Introduction: The cancellation of a surgery operation not only imposed too much expenditure on the healthcare system, but also waste lots of energy and time of patient and health care provider. The aim of this study was to assess reasons of surgery cancellation, and to provide a comprehensive list of cancellation reasons to implement in hospital information system for improving documentation.Method: This study is an applieddescriptive study which was conducted in two parts. In first part, all medical records of hospitalized patients in Shafa educational hospital during 6 months period which had been assigned a cancellation code based on ICD10, were assessed. The data were collected through a researchermade checklist and have been analyzed using SPSS.V18 software. The second part was literature review which provided a list of surgery cancellation reasons. To retrieve the relevant papers PubMed and Google Scholar databases were searched. Finally the results of two parts of the study were combined and a comprehensive list of surgery cancellation reasons was developed. Results: Of 7529 surgical operation, 1229 operations (16.4%) were cancelled. Most of the cancelled surgical operations were related to inappropriate patient rsquo;s clinical condition before surgery. Extracted cancellation reasons were categorized into 8 groups. Totally a list of 54 reasons for cancellation was developed.Conclusion: Precise documentation of surgery cancellation reasons in each health care institution in order to determine the reasons and plan to prevent them is essential. Designing an information system with proper information content can support the above mentioned issues.
