بررسی فرآیند ثبت اطلاعات دارویی بیماران بیمارستان آموزشی مرجع در شهر کرمان با استفاده از رویکرد شش سیگما

مقدمه: افزایش شدید هزینه های درمانی به همراه کاهش روند حمایتی دولت از بیمارستان ها، سبب پررنگ شدن نقش سازمان های بیمه به عنوان اصلی ترین منبع درآمدی بیمارستان شده است. بخش مهمی از مطالبات بیمارستان ها، مربوط به اسناد دارویی سازمان بیمه خدمات درمانی است که پس از رسیدگی به علت نقص در فرآیند، مشمول کسورات شده و از مطالبات بیمارستان کسر می گردد.روش: مطالعه حاضر توصیفی و به صورت گذشته نگر انجام شد. در این پژوهش نمونه گیری انجام نشد و کل 30 پرونده ناقص بیماران بیمه سلامت (ایرانیان) بیمارستان مرجع وارد مطالعه گردید. اطلاعات چک لیست ها وارد نرم افزار اکسل نسخه 22 شد، با استفاده از نمودار استخوان ماهی علت ها تعیین و برای تحلیل کیفیت فرآیند، از مقیاس شش سیگما استفاده گردید.نتایج: خطاهای فرآیند در قالب 6 گروه شناسایی گردید که بیشترین میزان خطاها به ترتیب مربوط به مراحل کنترل دستور دارویی و واردکردن آن در کاردکس و درخواست دارو از طریق سیستم تحویل دارو بوده است. بر اساس سیگمای محاسبه شده سطح کیفیت فرآیند مورد بررسی متوسط ارزیابی گردید. بیشترین میزان خطا مربوط به اسناد بیماران صندوق روستایی خدمات غیرگلوبال بود.نتیجه گیری: اقداماتی چون آموزش کارکنان، بازبینی و به روزرسانی سیستم ثبت و نگهداری اطلاعات و بهبود هماهنگی برون بخشی می تواند تا حد زیادی خطاها را در سه مرحله فوق کاهش دهد.

The Analysis of the Quality of Patient Drug Information Recording Process at Referral Teaching Hospital Using Six Sigma

Introduction: Sharp rise in health care costs associated with the reduction of hospitals' support by government has increased the importance of insurance organizations as the main source of hospitals' income. An important part of hospitals rsquo; demands is related to the medical records of Iranian Health Insurance organization that are subject to deductions and are deduced from the hospitals rsquo; demands after investigating the cause of defect in the process.Methods: The present study was conducted as a retrospective descriptive research. In this study, sampling was not conducted and all incomplete files of patients covered by Iranian Health Insurance Organization in the reference hospital (n=30) were entered into the study. Data of checklists were entered into Excel software version 22 and the causes were determined by using Fishbone diagram. Sixsigma scale was used to analyze the quality of the process.Results: Process errors were detected in 6 groups and the highest error rates were respectively related to pharmaceutical orders control stages, entering them in cards and requesting drug through drug delivering system. The quality of the reviewed process was evaluated as moderate level based on the calculated sigma. The highest rate of error was related to the records of patients covered by rural fund of nonglobal services.Conclusion: Measures such as staff training, monitoring and updating the record keeping system and improving external parts coordination can greatly reduce errors in three abovementioned stages.
