بررسی ارتباط آمادگی بیماران برای ترخیص از دیدگاه پرستاران با پیامدهای بعد از ترخیص

زمینه و هدف: سیستم های درمانی علاوه بر توجه به کمک به بهبود سلامتی بیماران حین بستری لازم است به پیامدهای بعد از ترخیص آنها نیز توجه نمایند. لذا شناسایی عوامل مرتبط با پیامدهای بعد از ترخیص، حائز اهمیت میباشد. مطالعه حاضر با هدف تعیین ارتباط آمادگی بیماران برای ترخیص از دیدگاه پرستاران با پیامدهای بعد از ترخیص انجام شد.روش کار: مطالعه حاضر از نوع توصیفی- تحلیلی بود که در سال 1401 انجام شد. جامعه پژوهش، بیماران بزرگسال بستری در بخشهای عمومی دو مرکز منتخب آموزشی- درمانی شهر سنندج و پرستاران هر کدام از بیماران در روز ترخیص بودند که از بین آنها تعداد 360 نفر با روش نمونه گیری در دسترس انتخاب شدند. ابزار جمع آوری اطلاعات شامل فرم اطلاعات دموگرافیک، پرسشنامه آمادگی ترخیص از دیدگاه پرستار و چک لیست 6 سوالی پیامدهای بعد از ترخیص بود. دادهها با نرم‌افزار spss-21 در سطح معناداری 0/05 و با آمار توصیفی و آزمون‌های آماری تی مستقل، کای اسکوئر، مدل رگرسیون چندگانه مورد تجزیه و تحلیل قرار گرفت. یافته‌ها: میانگین نمره کل آمادگی ترخیص بیماران، 1/19±7/82و در سطح متوسط بود. کمترین نمره آمادگی مربوط به بعد دانش (1/60±7/61) بود. بهبودی بعد از یک ماه در 93/4 درصد بیماران گزارش شد. بیمارانی که در فاصله ترخیص تا یک ماه بعد فوت شده بودند به طور معناداری نمره آمادگی حین ترخیص کمتری از دیدگاه پرستاران، دریافت کردند (0/001

investigating the relationship between patients' discharge readiness from the nurses' perspective and post-discharge outcomes

background: the healthcare systems, in addition to focusing on improving patient health during hospitalization, should pay attention to the outcomes followed by discharge. therefore, it is crucial to identify factors associated with post-discharge outcomes. hence, this study aimed to investigate the relationship between patients’ discharge readiness from the nurses’ perspective and post-discharge outcomes.methods: the present descriptive-analytical study was conducted in 2022. the study population included adult patients hospitalized in the general wards of two selected educational medical centers in sanandaj city and the discharge nurses of each patient. a total of 360 people were selected through convenience sampling. data collection tools included demographic information forms, a questionnaire on discharge readiness from the nurses’ perspective, and a 6-item checklist on post-discharge outcomes. data were analyzed using spss 21 software with a significance level of 0.05, using descriptive statistics and independent t-test, chi-square test, and multiple regression analysis.results: the total score of patients’ discharge readiness was 7.82±1.19 considered moderate. the lowest score was in the knowledge dimension (7.61±1.60). one month after discharge recovery was reported by 93.4% of patients. patients who died within one month after discharge had significantly lower discharge readiness scores from the nurses’ perspective (p<0.001).conclusion: these results indicated that although the overall discharge readiness score was in an appropriate range, the score in the knowledge dimension needs more attention. on the other hand, to reduce negative outcomes after discharge, evaluating the nurses’ assessment of the patient’s condition at the time of discharge and considering it in the discharge decision-making process should be taken into account, as patients who died had significantly lower discharge readiness from the nurses’ perspective
