بررسی تاثیر عصاره هیدروالکلی گیاه کیسه کشیش بر پس‫ دردهای زایمانی و حجم خونریزی: یک کارآزمایی بالینی تصادفی

زمینه و هدف: پس درد یا انقباضات دردناک رحمی پس از زایمان از مشکلات شایع بعد از زایمان است که پیامدهای مضری برای مادر و نوزاد دارد. امروزه استفاده از گیاهان دارویی و طب سنتی در کاهش پس درد مدنظر قرار گرفته است. در این کارآزمایی بالینی، تاثیر عصاره هیدروالکلی گیاه کیسه کشیش بر پس دردها و خونریزی‫های زایمانی مورد بررسی قرار گرفت. روش کار: در یک کارآزمایی بالینی سه کور شاهددار تصادفی شده، تعداد 110 نفر از مادران با زایمان طبیعی در بیمارستان ولایت دامغان به روش نمونه گیری در دسترس انتخاب و به روش تصادفی به دو گروه مداخله و کنترل تقسیم شدند. در گروه مداخله علاوه بر کپسول مفنامیک اسید 250 میلی‫گرمی خوراکی، قطره زیرزبانی عصاره هیدروالکلی گیاه کیسه کشیش داده شد. گروه کنترل قطره زیرزبانی پلاسبو و کپسول مفنامیک اسید 250 میلی‫گرمی خوراکی را دریافت نمودند. پس درد زایمانی با مقیاس دیداری درد در 6 ساعت اول، هر یک ساعت ارزیابی شده و سپس هر 6 ساعت یک بار تا 24 ساعت اول پس از زایمان اندازه گیری و ثبت شد. داده ها با استفاده از نرم افزار spss-20 و آزمون‫های آماری تی مستقل، من- ویتنی، کای دو و آنالیز واریانس با اندازه گیری مکرر مورد تجزیه و تحلیل قرار گرفتند. یافته ها: حجم خونریزی ساعت دوم و سوم در گروه مداخله به طور معنی داری کمتر بود. میانگین شدت درد در ساعات 24،18،12،6،5،3،2،1 در گروه مداخله کمتر از گروه کنترل بود. فقط در شدت درد ساعت چهارم، بین دو گروه تفاوت معنی دار وجود نداشت (0.830=p). تحلیل واریانس نشان داد که شدت درد در زمان‫های مختلف در گروه مداخله با گروه کنترل تفاوت معنی‫داری داشته و در گروه مداخله کمتر بود (0.026=p).نتیجه گیری: علیرغم تاثیر زمان در کاهش شدت پس درد زایمانی، شدت درد در زمان های مختلف در گروه مداخله به‫طور معنی داری کمتر بود، پس می‫توان گفت تجویز عصاره گیاه کیسه کشیش بر کاهش شدت پس دردهای زایمانی موثر است.

effect of hydro-alcoholic extract of capsella bursa pastoris on postpartum pain and volume of bleeding, a randomized clinical trial

background & aim: postpartum pain or painful uterine contractions are one of the most common postpartum problems. this pain is affected by various factors and has maternal and neonatal adverse consequences. nowadays, herbal and traditional remedies have been considered for management of this pain. the aim of this study was to evaluate the effect of hydroalcoholic extract of priest sachet capsella bursa-pastoris on postpartum pain and volume of bleeding in women referring to damghan hospital affiliated to semnan university of medical science.method: in this clinical trial, 110 mothers with vaginal delivery were enrolled. inclusion criteria were postpartum pain criteria of four and more. patients were randomly assigned into two groups, the first group received hydroalcoholic extract of capsella bursa-pastoris and the other received placebo. visual scale of pain was used to assess postpartum pain. data were analyzed by spss-20 software using independent t-test, mann-whitney, chi-square and analysis of variance with repeated measures and linear regression at significance level less than 0.05.results: the mean age was 29.13±6.6 years. the two groups were matched in terms of age and other factors including cultural and economic status, parity, delivery, and abortion, history of stillbirth and curettage, anthropometric indices, administration of oxytocin during labor, labor conditions and neonatal factors. there was no significant difference between the two groups in receiving of analgesics. analysis of variance showed that time had a significant effect on pain severity, however, pain intensity at different times in the intervention group was significantly lower than control group (p=0.026).conclusion: despite the effect of time on decreasing of postpartum pain, the severity of pain at different times in the intervention group was significantly lower than control group and recommended for postpartum pain.
