ارتباط جو اخلاقی با مراقبت‫ های پرستاری فراموش شده از دیدگاه بیماران مبتلا به سرطان و پرستاران بخش ‫های انکولوژی شهرهای زنجان و رشت

زمینه و هدف: کیفیت بالای مراقبت پرستاری از بیماران مبتلا به سرطان یک اولویت مشترک برای سیاست گذاران عرصه سلامت است. دو متغیر تاثیرگذار مهم بر کیفیت مراقبت‫ های پرستاری؛ جواخلاقی حاکم بر بیمارستان ‫ها و مراقبت ‫های پرستاری فراموش شده است. این مطالعه با هدف تعیین ارتباط وضعیت جواخلاقی و مراقبت‫ های پرستاری فراموش شده در بخش های ارائه دهنده خدمات به بیماران مبتلا به سرطان در دو شهر رشت و زنجان انجام شد. روش کار: این مطالعه مقطعی در سال 1399 انجام شد. تعداد مشارکت‌کنندگان، 360 بیمار مبتلا به سرطان و 84 پرستار شاغل در بخش های انکولوژی دو شهر رشت و زنجان بودند که با استفاده از روش نمونه‌گیری توزیع پواسون وارد مطالعه شدند. ابزار جمع‌آوری داده‌ها دو مقیاس استاندارد مراقبت های فراموش شده و جو اخلاقی و دو فرم مشخصات جمعیت شناختی مختص بیماران و پرستاران بود. جهت تجزیه‌و تحلیل داده‌ها از نرم‌افزار آماری spss-16 و آزمون‫ های تی مستقل و همبستگی پیرسون استفاده شد. یافته ها: نتایج نشان داد که جو اخلاقی غالب در واحدهای مراقبتی یادشده، جو حرفه گرا و قانون مدار بوده و جو اخلاقی مستقل در رتبه انتهایی قرار داشت. میانگین نمره مراقبت‌های پرستاری فراموش شده، 0/40±1/95 از 5 بود. بین جو اخلاقی و مراقبت‌های پرستاری فراموش شده در واحدهای تحت مطالعه، ارتباط منفی و معنی داری مشاهده شد (r=0.422, p<0/001). نتیجه گیری: وجود ارتباط منفی و معنی دار بین جو اخلاقی و مراقبت‌های پرستاری فراموش شده نشان دهنده این بود که جو اخلاقی و قانون مداری و حرفه گری پرسنل با کاهش میزان بروز مراقبت‌های پرستاری فراموش شده ارتباط معناداری دارد. به نظر می‫رسد با تهیه و تنظیم گایدلاین های مراقبتی، قوانین و دستورالعمل ‫های شفاف مراقبتی بتوان وضعیت مراقبت ‫های پرستاری را بهبود بخشید.

relationship between ethical climate and missed nursing care from the point of view of patients with cancer and nurses working in oncology wards of zanjan and rasht cities

background & aim:  the high quality of nursing care for cancer patients is a shared priority for health policymakers. two important influencing variables on the quality of nursing care are the ethics climate governing hospitals and missed nursing care. this study aimed to determine the relationship between ethical climates and missed nursing care in centers providing services to cancer patients in rasht and zanjan.methods: this crosssectional study was conducted in 2019. the participants were 360 patients with cancer and 84 nurses working in the oncology wards of rasht and zanjan, who were included in the study using the poisson distribution sampling method. the data collection tools were two standard scales; the miss care survey and ethical climate questionnaire, and two demographic forms specific to patients and nurses. spss version 16 statistical software and independent ttests, and pearson’s correlation were used to analyze the data. results: the results showed that the dominant ethical climate in the mentioned care units was professional and laworiented, and the independent ethical climate was at the bottom. the mean score of missed nursing care is 1.95±0.40 out of 5. according to the pearson correlation test, there was a significant negative relationship between ethical climate and missed nursing care in cancer care units (r=0.422, p<0.001).conclusion: a negative and significant relationship between the ethical climate and missed nursing care showed that nurses’ correct perception of the ethical climate reduces missed nursing care. it seems that the nursing care quality can be improved by preparing and setting up care guidelines, rules and clear care instructions.
