Comparison between dilatation & curettage and Pipelle's endometrial sampling for safer evaluation in postmenopausal bleeding women

Aim: to compare dilatation & curettage and pipelle endometrial sampling for safer evaluation in postmenopausal bleeding women. study design: it was comparative study. duration of study: the study was conducted from march 2009 to september 2009. settings: department of obstetrics and gynaecology,jubilee female hospital/bv hospital,bahawalpur. methods: a total of 100 patients of postmenopausal bleeding reporting to the out patient department of gynecology and obstetrics,b v hospital,bahawalpur were included by random sampling technique while women with extra-genital origin of bleeding i.e.,from bladder,urethra,rectum and anus,woman who were taking hormone replacement therapy and women who have premature ovarian failure syndrome under age of 45 years were excluded. out of these 50 patients were allotted for pipelle's biopsy (group-a) and in other 50 patients dilatation and curettage was performed under g/a (group b). results: out of 100 cases 42(42%) were between 45-55 years,34(34%) were between 56-65 years,and 24(24%) were between 66-75 years of age,mean+sd was 58.89+8.21 years,52(52%) had malignant lesions,44(44%) benign and only 4% were un-determined. complications of each procedure were recorded as 9(18%) infection,5(10%) hemorrhage,2(4%) perforation while in 1(2%) case had complications of g/a in the d&c group and no complications were recorded in patients allotted for pipelle's biopsy,p value was calculated as 0.000 which shows a significant difference in both groups. conclusion: pipelle's endometrial sampling is a safer method of evaluating postmenopausal bleeding as compared to d&c. pipelle's sampling is an inexpensive outpatient procedure.
